目的对酶联免疫吸附试验(ELISA)诊断日本血吸虫人群感染的效能进行系统评价和meta分析。方法在中国知网(CNKI)、万方数据(Wanfangdata)、重庆维普(VIP)、PubMed、Web of Science、Scopus和Google Scholar中检索有关ELISA法诊断日本血吸虫人群感染的全部文献,应用R语言进行meta分析,综合评价ELISA法诊断日本血吸虫人群感染的效能。结果共有9篇符合纳入和排除标准的文献纳入meta分析,文献质量较高。纳入的文献中,ELISA法诊断日本血吸虫人群感染灵敏度为57.1%~97.3%,特异度为20.4%~84.3%;采用随机效应模型加权合并的敏感度、特异度、阳性似然比和阴性似然比分别为84.9%、50.4%、1.666和0.367。应用加权最小二乘法、一般最小二乘法和稳健法计算得出的ELISA合并准确度分别为0.650、0.686和0.666,合并OR值分别为3.432、4.784和3.959,3种方法拟合的ELISA诊断试验SROC曲线下面积分别为0.695、0.741和0.715,加权合并DOR值为4.78。结论ELISA对日本血吸虫人群感染的诊断效能较高,可望为我国消除血吸虫病阶段提供一种值得信赖的免疫诊断工具。
Objective To systematically evaluate and analyze the efficacy of enzyme-linked immunosorbent assay (ELISA) in the diagnosis of Schistosoma japonicum infection. Methods All the literature on ELISA for the diagnosis of Schistosoma japonicum infection by ELISA was searched in CNKI, Wanfangdata, VIP, PubMed, Web of Science, Scopus and Google Scholar. The R language meta-analysis, comprehensive evaluation of the ELISA method of Schistosoma japonicum infection in the crowd. Results A total of 9 articles that met the inclusion and exclusion criteria were included in the meta-analysis and the literature was of high quality. In the included literature, the sensitivity, specificity, positive likelihood ratio and negative likelihood ratio of the combined diagnosis of Schistosoma japonicum infection were 57.1% -97.3% and 20.4% -84.3% respectively. Compared to 84.9%, 50.4%, 1.666 and 0.367 respectively. The accuracy of ELISA with weighted least square method, general least squares method and robust method were 0.650,0.686 and 0.666 respectively, and the combined OR values were 3.432, 4.784 and 3.959, respectively. The three methods of fitting ELISA diagnostic test SROC Area under the curve were 0.695,0.741 and 0.715 respectively, and the weighted combined DOR value was 4.78. Conclusion ELISA has a high diagnostic efficacy against Schistosoma japonicum infection and is expected to provide a reliable immunodiagnostic tool for the elimination of schistosomiasis in our country.
