目的探讨高渗盐水治疗大面积脑梗死继发脑水肿、颅高压的价值研究。方法 96例大面积脑梗死合并脑水肿、颅高压患者,随机分为对照组和观察组,每组48例。对照组采用甘露醇进行治疗,观察组采用高渗盐水进行治疗,对所有患者在治疗前后采用无创颅内压(IPC)监测仪监测患者的一系列指标,1周之后进行格拉斯哥昏迷(GCS)评分。结果患者的各项指标治疗后比较,观察组优于对照组(P<0.05)。对照组显效30例,有效4例,无效14例,总有效率为70.8%;观察组显效44例,有效3例,无效1例,总有效率为97.9%。观察组的治疗效果明显优于对照组(P<0.05)。结论高渗盐水对治疗大面积脑梗死继发脑水肿、颅高压的治疗效果比较好,可以作为降低颅高压的用药在临床上推广使用。
Objective To investigate the value of hypertonic saline in treating cerebral edema and intracranial hypertension following large area cerebral infarction. Methods A total of 96 cases of cerebral infarction with cerebral edema and patients with intracranial hypertension were randomly divided into control group and observation group, 48 cases in each group. The control group was treated with mannitol. The observation group was treated with hypertonic saline. Before and after treatment, all patients were monitored by a series of indexes of noninvasive intracranial pressure (IPC) monitors. After one week, the scores of Glasgow coma (GCS) . Results After treatment, the indexes in the observation group were better than those in the control group (P <0.05). In the control group, 30 cases were markedly effective, 4 cases effective, 14 cases ineffective, and the total effective rate was 70.8%. In the observation group, 44 cases were markedly effective, 3 cases were effective, and 1 case was ineffective. The total effective rate was 97.9%. Observation group’s treatment effect was significantly better than the control group (P <0.05). Conclusion Hypertonic saline treatment of cerebral infarction secondary to cerebral edema, the treatment of intracranial hypertension is relatively good, can be used as medicine to reduce intracranial hypertension in clinical practice.
