目的:检测前列腺癌患者及对照者外周血单个核细胞前列腺特异性抗原(PSA)mRNA表达,并探讨其临床意义。方法:选择39例前列腺癌患者,12例健康体检者作为对照,取5 mL静脉血,肝素抗凝,Ficoll密度梯度离心分离外周血单个核细胞,RT-PCR检测PSA mRNA表达。结果:①39例前列腺癌患者外周血单个核细胞PSA mRNA表达阳性检出率66.7%(26/39),其中血清PSA>20.0μg.L-1者23例,PSA mRNA表达阳性检出率87.9%(20/23);血清PSA≤20.0μg.L-1者16例,PSA mRNA表达阳性检出率37.5%(6/16)。对照组未见阳性表达。②16例血清PSA≤20.0μg.L-1的前列腺癌患者PSA mRNA阳性表达的Gleason评分为:1~4分0/3,5~7分1/8,8~10分5/5,6例阳性者均为中、低分化癌。结论:外周血单个核细胞PSAmRNA的检测在血清PSA>20.0μg.L-1的前列腺癌患者中具有较高的敏感度,是一种可靠的检测方法。血清PSA≤20.0μg.L-1的前列腺癌患者外周血单个核细胞PSA mRNA的检测可以预示微转移的发生。
Objective: To detect the expression of prostate specific antigen (PSA) mRNA in peripheral blood mononuclear cells of patients with prostate cancer and its controls and to investigate its clinical significance. Methods: Thirty-nine patients with prostate cancer and 12 healthy controls were selected as control. Peripheral blood mononuclear cells were isolated by 5 mL venous blood, heparin anticoagulation and Ficoll density gradient centrifugation. The expression of PSA mRNA was detected by RT-PCR. Results: (1) The positive rate of PSA mRNA expression in peripheral blood mononuclear cells was 66.7% (26/39) in 39 patients with prostate cancer, including 23 cases with serum PSA> 20.0μg.L-1, the positive rate of PSA mRNA was 87.9% (20/23). Serum PSA was less than or equal to 20.0μg.L-1 in 16 cases. The positive rate of PSA mRNA expression was 37.5% (6/16). No positive expression in the control group. ② The Gleason score of PSA mRNA expression in 16 cases of prostate cancer patients with serum PSA≤20.0μg.L-1 was 1 ~ 4 points 0/3, 5 ~ 7 points 1/8 and 8 ~ 10 points 5/5 cases Positive were low-grade cancer. Conclusion: The detection of PSA mRNA in peripheral blood mononuclear cells is a reliable method for detecting prostate cancer in patients with PSA> 20.0μg.L-1. Detection of PSA mRNA in peripheral blood mononuclear cells of prostate cancer patients with PSA≤20.0μg.L-1 can predict the occurrence of micrometastasis.
