目的:评估联合应用极速成像技术与脉象仪检测原发性高血压病患者动脉弹性功能的价值。方法:纳入102例原发性高血压病患者作为高血压组,125例健康志愿者作为对照组。应用极速成像技术检测颈动脉脉搏波传导速度(pulse wave velocity,PWV),包括收缩期起始(beginning of the systole,BS)时及收缩期结束(ending of the systole,ES)时的PWV;应用脉象仪行桡动脉脉象参数检测,包括h3、h4、h5、h3/h1、h4/h1、h5/h1、w1、w2、w1/t、w2/t;采用Spearman非参数法对两种技术参数进行相关性分析。结果:BS和ES时,高血压组的PWV均显著高于对照组(P<0.01)。脉象参数中,高血压组的h3、h4、h3/h1、w2、w1/t、w2/t较对照组增高,差异有统计学意义(P<0.01,P<0.05)。对照组BS、ES时的PWV与h3/h1、h4/h1呈正相关(P<0.05),高血压组BS、ES时的PWV与h4、h3/h1、h4/h1呈正相关(P<0.05)。结论:极速成像技术结合脉象仪能够客观评估原发性高血压病患者的动脉弹性功能,且两种技术参数间具有良好的相关性。
OBJECTIVE: To assess the value of combined use of speed imaging and pulse detector in detecting arterial elasticity in patients with essential hypertension. Methods: A total of 102 patients with essential hypertension were included as hypertension group and 125 healthy volunteers as control group. Carotid artery pulse wave velocity (PWV) was measured using a speed imaging technique, including PWV at the beginning of the systole (BS) and ending of the systole (ES) Pulse arteries were measured with radial artery pulse parameters, including h3, h4, h5, h3 / h1, h4 / h1, h5 / h1, w1, w2, w1 / t and w2 / t; Spearman nonparametric method was used to measure the two technical parameters Conduct correlation analysis. Results: PWV in hypertension group was significantly higher than that in control group at BS and ES (P <0.01). In the pulse parameters, h3, h4, h3 / h1, w2, w1 / t and w2 / t of hypertensive group were higher than those of the control group, the difference was statistically significant (P <0.01, P <0.05). There was a positive correlation between PWV and h3 / h1, h4 / h1 in BS and ES of control group (P <0.05), while PWV in BS and ES of hypertension group was positively correlated with h4, h3 / h1 and h4 / . CONCLUSION: The combination of speed imaging and pulse meridian can evaluate the arterial elasticity of patients with essential hypertension objectively and has a good correlation between the two technical parameters.
