目的:通过血药浓度监测,指导急性淋巴细胞白血病患儿的大剂量甲氨蝶呤化疗和解救方案,保证用药安全有效。方法:18例急性淋巴细胞白血病患儿,接受41次3,5g.m-2的大剂量甲氨蝶呤化疗,用荧光偏振免疫法测定甲氨蝶呤44,68h的血药浓度,指导亚叶酸钙的救援,并观察不良反应发生情况。结果:两剂量组的大部分患儿血药浓度处于安全范围内,无不可逆的不良反应发生。结论:甲氨蝶呤的血药浓度监测提高了用药安全性和有效性,患儿能够耐受5g.m-2的甲氨蝶呤剂量。
OBJECTIVE: To monitor and guide large-dose methotrexate chemotherapy and rescue in children with acute lymphoblastic leukemia through blood concentration monitoring to ensure safe and effective medication. Methods: 18 children with acute lymphoblastic leukemia received 41 doses of 3,5 g · m-2 methotrexate, and the blood concentration of methotrexate 44 and 68 h were measured by fluorescence polarization immunoassay Calcium folate rescue, and observe the occurrence of adverse reactions. Results: Most of the two dose groups of children with plasma concentrations within the safe range, no adverse reactions occur irreversible. Conclusion: The monitoring of methotrexate plasma concentration improves the safety and efficacy of the drug. Children can tolerate 5g.m-2 methotrexate dose.
