目的评价符合线路18FDG-SPECT(18FDG-SPECT with coincidence)在鉴别肺部良恶性病变中的作用。方法对86例胸部X线片或CT异常的患者进行18FDG-SPECT检查。18FDG-SPECT扫描结果经两位以上有经验的核医学科医生阅片进行定性分析,同时采用肺部病变/正常肺组织的18FDG摄取比值(T/N)对病变的良恶性进行鉴别。以病理学诊断作为金标准,SPECT扫描结果均与其进行比较。结果86例肺部病变中56例为恶性,30例为良性。对照术后病理检查结果18FDG-SPECT诊断的准确性、敏感性、特异性、阳性预测率和阴性预测率分别为94.2%,96.4%,90%,94.7%和93.1%。结论符合线路18FDG-SPECT检查能准确地鉴别肺部病变的良恶性,其诊断的准确性与18FDG-PET扫描相似,但与18FDG-PET相比,18FDG-SPECT具有价廉且易于开展的优点。
Objective To evaluate the role of line 18FDG-SPECT (18FDG-SPECT with coincidence) in the differential diagnosis of benign and malignant lung diseases. Methods Totally 86 patients with chest X-ray or CT abnormalities were examined by 18FDG-SPECT. The results of 18FDG-SPECT scan were qualitatively analyzed by two or more experienced nuclear medicine physicians. The pathological changes of benign and malignant lesions were also identified by the 18FDG uptake ratio (T / N) of lung lesions / normal lung tissues. Pathological diagnosis as the gold standard, SPECT scan results were compared with them. Results Of 86 cases of lung disease, 56 cases were malignant and 30 cases were benign. The accuracy, sensitivity, specificity, positive predictive value and negative predictive value of 18FDG-SPECT were 94.2%, 96.4%, 90%, 94.7% and 93.1% respectively. Conclusions The line 18FDG-SPECT can accurately identify the benign and malignant pulmonary lesions. The diagnostic accuracy is similar to that of 18FDG-PET scan. However, 18FDG-SPECT has the advantage of being cheap and easy to perform compared with 18FDG-PET.
