目的 :探讨周围型非小细胞肺癌 (NSCL C)的 CT征象与增殖细胞核抗原 (PCNA)表达的相关性。方法 :对 72例经手术病理证实的周围型 NSCL C病例术前进行 CT扫描 ,并对肿瘤标本用 SP法进行 PCNA免疫组化染色 ,用图像分析仪测量肿瘤细胞核 PCNA阳性表达分布面积。结果 :(1) CT显示瘤体最大直径 >3 cm者 ,深分叶征、棘状突起、坏死和空洞及纵隔淋巴结肿大者与PCNA阳性表达面积升高有关。 (2 )瘤体呈周围强化形的肺癌 PCNA阳性表达面积高于瘤体呈完全强化形的肺癌。 (3)肺癌的PCNA阳性表达面积与肿瘤分化程度呈负相关。结论 :周围型 NSCL C的 CT征象瘤体 >3 cm,深分叶征、棘状突起、坏死和空洞及纵隔淋巴结肿大与 PCNA阳性表达面积密切相关 ,具有更高的恶性程度 ,可作为推测预后的指标
Objective: To investigate the correlation between CT signs and the expression of proliferating cell nuclear antigen (PCNA) in peripheral non-small cell lung cancer (NSCL C). Methods: Seventy-two cases of peripheral NSCL C confirmed by surgery and pathology were preoperatively scanned by CT scan. PCNA immunohistochemical staining was performed on the tumor samples by SP method. The area of PCNA positive expression was measured by image analyzer. Results: (1) CT showed that the largest diameter of tumor> 3 cm, deep lobulation, spinous process, necrosis and cavity and mediastinal lymph nodes were associated with increased area of PCNA positive expression. (2) The positive expression area of PCNA in lung cancer with enhanced shape around the tumor was higher than that with completely enhanced shape in the tumor. (3) The positive expression area of PCNA in lung cancer was negatively correlated with the degree of tumor differentiation. Conclusion: CT findings of peripheral NSCL C> 3 cm, deep lobulation, spinous process, necrosis and cavity and mediastinal lymph node enlargement are closely related to the positive expression area of PCNA, with higher malignant degree, which can be used as speculation Prognosis index
