目的探讨类风湿关节炎(Rheumatoid arthritis,RA)病人发生心血管病变(Cardiovascular diseases,CVD)的非传统危险因素。方法选取河北省人民医院就诊的143例类风湿关节炎病人,将其分为RA合并CVD组及单纯RA组,检测各组血清脂蛋白相关磷脂酶A2(Lipoprotein associated phospholipase A2,Lp-PLA2)、同型半胱氨酸(Homocysteine,Hcy)、超敏C反应蛋白(hs-CRP)、纤维蛋白原(FIB)、尿酸(UA)、类风湿因子(RF)、抗环瓜氨酸肽抗体(ACCP)的水平,采用SPSS 17.0统计软件进行数据整理并分析Lp-PLA2、Hcy、hs-CRP、UA、RF、ACCP在两组间的差异,并采用logistic回归分析以上指标是否为RA病人伴发CVD的危险因素。结果 RA合并CVD组的Lp-PLA2、Hcy、hs-CRP、RF明显高于单纯RA组(P<0.05),回归分析显示Lp-PLA2、Hcy、CRP、RF为RA病人伴发CVD的危险因素。结论 RA合并CVD病人血清LpPLA2、Hcy、hs-CRP、RF水平较单纯RA病人显著升高,并与RA伴发CVD密切相关。
Objective To investigate the non-traditional risk factors of cardiovascular disease (CVD) in patients with rheumatoid arthritis (RA). Methods A total of 143 rheumatoid arthritis patients treated by Hebei Provincial People’s Hospital were divided into two groups: RA combined with CVD group and simple RA group. The levels of serum lipoprotein associated phospholipase A2 (Lp-PLA2) Homocysteine (Hcy), hs-CRP, fibrinogen (FIB), uric acid (UA), rheumatoid factor (RF), anti-cyclic citrullinated peptide antibody ), SPSS 17.0 statistical software was used to analyze the data and analyze the differences of Lp-PLA2, Hcy, hs-CRP, UA, RF and ACCP between the two groups. Logistic regression analysis was used to analyze whether the above indexes were associated with CVD Risk factors. Results The levels of Lp-PLA2, Hcy, hs-CRP and RF in RA combined with CVD group were significantly higher than those in RA group (P <0.05). Regression analysis showed that Lp-PLA2, Hcy, CRP and RF were the risk factors of CVD with RA . Conclusions Serum levels of LpPLA2, Hcy, hs-CRP and RF in patients with RA combined with CVD are significantly higher than those in patients with RA alone and are closely related to the association with RA.
