为揭示手指屈肌腱鞘滑车系统的形态特点,探讨滑车的形态、分布与功能之间的相互关系,对30只固定手的120个手指进行了巨微解剖观测,观测内容包括:滑车系统的构成、各滑车的形态特点、附着部位、宽度及各滑车的体表定位。结果显示:滑车系统由5个环行滑车(A1~A5)、4个交叉滑车(C0~C3)和1个掌腱膜滑车(PA滑车)构成;环行滑车几乎都恒定存在,形态变异很小,而交叉滑车不很恒定,形态变异也比较大;A2、A4滑车主要附着于近节、中节指骨体,A1、A3、A5滑车分别主要附着于掌指关节(MP)、近侧指间关节(PIP)和远侧指间关节(DIP)掌板,交叉滑车通常一端附着于掌板,一端附着于指骨;A2滑车是最宽的滑车,平均宽度16.8mm,其次是PA滑车、A1滑车、A4滑车,A3、A5滑车及交叉滑车均较窄,在2~4mm范围内;A2远端和A4中部是腱鞘最厚的部位。分析滑车的形态特点,得出以下结论:环形滑车对指屈肌腱的约束作用大于交叉滑车;环形滑车中,A2和A4滑车对肌腱的约束作用大于A1、A3、A5滑车。
To reveal the morphological characteristics of the flexor tendon sheath system of the finger flexor, the relationship between the shape, distribution and function of the tackle was explored. The microdissection of 120 fingers of 30 fixed hands was observed. The observations included: , The characteristics of each pulley shape, attachment site, width and surface positioning of each pulley. The results showed that the trolley system consisted of five circular trolleys (A1 ~ A5), four cross trolley (C0 ~ C3) and one palm apophysis trolley (PA trolley) While A2 and A4 are mainly attached to the proximal and middle phalanges. A1, A3 and A5 are mainly attached to the metacarpophalangeal joint (MP) and proximal interphalangeal joint (PIP), and the distal interphalangeal joint (DIP) palmtop. Crossovers are usually attached to the palm plate at one end and attached to the phalanges on one end. The A2 block is the widest block with an average width of 16.8mm, followed by the PA block, the A1 block , A4 pulley, A3, A5 pulley and cross pulley are narrow, in the range of 2 ~ 4mm; A2 distal and A4 middle is the most thick tendon sheath site. Analysis of the characteristics of the shape of the pulley, draw the following conclusions: The ring pulley on the flexor tendon restraint greater than the cross pulley; ring block, the A2 and A4 pulley restraint greater than the tendon of A1, A3, A5 pulley.
