目的评价超选择动脉溶栓及经皮腔内血管成形术治疗急性缺血性脑中风的临床疗效。方法对 3 0例急性缺血性脑中风患者 ,经股动脉或颈动脉插管 ,用微导丝将微导管送至血管闭塞部位 ,注入尿激酶行溶栓治疗 ,对 6例颅外段血管狭窄者 ,采用经皮腔内血管成形术消除 ,并注入罂粟碱扩张颅内血管。结果血管完全再通 15例 ,部分再通 12例 ,治疗 3月后日常生活能力指数评分 :10 0分 2 0例 ,75~ 95分 6例 ,占 86.67%。结论早期超选择动脉溶栓配合经皮腔内血管成形术和罂粟碱可明显提高闭塞血管再通率 ,是目前治疗急性脑梗死有效的方法。
Objective To evaluate the clinical effects of transcatheter arterial thrombolysis and percutaneous transluminal angioplasty in the treatment of acute ischemic stroke. Methods Thirty patients with acute ischemic stroke were intubated via the femoral artery or carotid artery. The microcatheter was delivered to the site of vascular occlusion by micro-guide wire. Urokinase was administered to thrombolytic therapy. Six cases of extracranial vascular Stenosis, the use of percutaneous transluminal angioplasty to eliminate, and papaverine expansion of intracranial blood vessels. Results The blood vessels were completely recanalized in 15 cases and partial recanalization in 12 cases. After 3 months of treatment, the index of daily living index was 10 0 20 cases, 75 95 cases 6 cases, accounting for 86.67%. Conclusion Early superselective arterial thrombolysis combined with percutaneous transluminal angioplasty and papaverine can significantly improve the occlusion vessel recanalization rate, which is an effective method for the treatment of acute cerebral infarction.
