目的:观察牙周基础治疗对慢性牙周炎(CP)患者种植义齿修复后龈沟液(PICF)等指标的影响。方法:选择在我院行种植义齿修复1~2年的稳定期CP上下颌后牙缺损52例共67颗种植义齿作为观察组,根据临床检查分为健康种植体组(HI+CP组)25颗、种植体周围黏膜炎组(MI+CP组)42颗[4];另选择在我院行前磨牙、磨牙区种植义齿修复1~2年的牙周健康28例共33颗种植义齿作为对照组(HI组)。分别检测治疗前后种植义齿探诊深度(PD)、改良菌斑指数(mPLI)及改良出血指数(mSBI)等指标,采集PICF标本,以酶联免疫吸附法检测PICF量及其中白细胞介素8(IL-8)、肿瘤坏死因子α(TNF-α)水平。观察比较两组种植义齿修复后各指标的变化。结果:治疗前,MI+CP组mPLI、mSBI、PICF量、IL-8、TNF-α水平等指标与HI组比较,均差异非常显著(P<0.01);HI+CP组上述指标与HI组比较,均差异显著(P<0.05);两组PD与HI组比较,差异不显著(P>0.05),X线检查未见骨吸收超过2mm。经系统牙周基础治疗后,3组PICF量均较治疗前显著减少(P<0.05),MI+CP组、HI+CP组IL-8、TNF-α水平均较治疗前非常显著下降(P<0.01),MI+CP组mPLI、mSBI和HI+CP组mPLI均较治疗前显著改善(P<0.05)。治疗后,MI+CP组mPLI、mSBI、PICF量、TNF-α水平等指标与HI组差异显著(P<0.05),IL-8水平与HI组差异非常显著(P<0.01);HI+CP组仅IL-8、TNF-α水平与HI组差异显著(P<0.05),mPLI、mSBI、PICF量与HI组差异不显著(P>0.05)。结论:牙周基础治疗有利于改善CP患者种植修复后义齿周围组织健康。
Objective: To observe the effects of basic periodontal treatment on indexes of gingival crevicular fluid (PICF) and other indexes in patients with chronic periodontitis (CP). Methods: A total of 67 denture prostheses, consisting of 67 denture prostheses, were selected as observational group and divided into healthy implant group (HI + CP group) 25 42 patients with mucositis around the implant (MI + CP group) [4]. In addition, 28 cases of periodontal health improvement of periodontal health with implant denture and molars in 33 patients Control group (HI group). (PD), modified plaque index (mPLI) and modified hemorrhage index (mSBI) were measured before and after treatment. PICF specimens were collected and the levels of PICF and interleukin-8 IL-8), tumor necrosis factor alpha (TNF-alpha) levels. The changes of the indexes of the two groups of implants were observed and compared. Results: The levels of mPLI, mSBI, PICF, IL-8 and TNF-α in MI + CP group were significantly different from those in HI group before treatment (P <0.01) (P <0.05). There was no significant difference between PD group and HI group (P> 0.05). There was no bone resorption more than 2mm in X-ray examination. After systemic periodontal treatment, the PICF levels in three groups were significantly decreased (P <0.05), while the levels of IL-8 and TNF-α in MI + CP group and HI + CP group were significantly decreased compared with those before treatment (P <0.01). The mPLI of mPLI group, mSBI group and HI + CP group in MI + CP group were significantly improved compared with that before treatment (P <0.05). After treatment, the levels of mPLI, mSBI, PICF and TNF-α in MI + CP group were significantly different from those in HI group (P <0.05) The levels of IL-8 and TNF-α in the group were significantly different from those in the HI group (P <0.05). The levels of mPLI, mSBI and PICF were not significantly different from those in the HI group (P> 0.05). Conclusion: The basic periodontal treatment can improve the health of the peri-implant tissue after CP implantation.
