目的 探讨超声、CT及MRI检查在眼上静脉扩张及其病因诊断中的意义。方法 对1984~ 2 0 0 0年临床资料完整的 116例 (2 32只眼 )伴眼上静脉扩张患者进行回顾性分析。结果 发现多种疾病均可引起眼上静脉扩张 ,颈动脉 海绵窦瘘是导致眼上静脉扩张的主要疾病。其中颈动脉 海绵窦瘘 92例 (包括高和低流瘘 ) ,眼型Graves病 14例 ,眶尖炎症 2例 ,Tolosa Hunt综合征 2例 ,眼部血管畸形 2例 ,眶炎性假瘤、眶内血肿、海绵窦肿瘤及海绵窦血栓形成各 1例。扩张的眼上静脉直径为 3 5~ 7 0mm ,各病种眼上静脉管径扩张大小无明显差异 ,但颈动脉 海绵窦瘘眼上静脉最粗 7 0mm。影像学检查同时能发现眼外肌肥大、眶部病变及海绵窦膨大等相关影像学表现。结论 超声、CT及MRI检查均能良好显示眼上静脉扩张 ,综合其他影像学表现 ,可确定眼上静脉扩张的病因
Objective To investigate the significance of ultrasonography, CT and MRI in the diagnosis of dilated orbital dilation and its etiology. Methods A retrospective analysis was performed on 116 cases (322 eyes) with dilation of the eye vein from 1984 to 2000. The results showed that a variety of diseases can cause dilation of the eye vein, carotid cavernous fistula is the leading cause of dilation of the eye’s major diseases. Among them, 92 cases of carotid cavernous fistula (including high and low fistula), 14 cases of ocular Graves disease, 2 cases of orbital inflammation, 2 cases of Tolosa Hunt syndrome, 2 cases of ocular vascular malformations, orbital inflammatory pseudotumor, Orbital hematoma, cavernous sinus tumor and cavernous sinus thrombosis in 1 case. The diameter of the dilated suprachoroidal vein was 35-70 mm. There was no significant difference in the size of the dilation of the supra-vein vein between the various diseases. However, the diameter of the upper eye vein in the carotid-cavernous fistula was 70 mm. Imaging studies can also find extraocular muscle hypertrophy, orbital lesions and cavernous sinus enlargement and other related imaging findings. Conclusion Ultrasound, CT and MRI examination can all show the superior dilation of the superior ophthalmic vein. Combined with other imaging findings, the etiology of dilated orbital dilation can be determined
