目的探讨每日补充铁与间歇性补充铁对学龄前儿童生长发育和铁营养状况的影响差异。方法选取某幼儿托管机构的424名3~6岁学龄前儿童为研究对象,随机抽样法将其分成每日补铁组(n=133)、间歇性补铁组(n=133)和对照组(n=158)三组。行为期3个月的随访,比对三组受试幼儿在体重、身高等生长发育指标增加值及血清铁蛋白(SF)、血红蛋白(H b)、血清运铁蛋白受体(s Tf R)、红细胞游离原卟啉(FER)等营养指标增加/降低值上的差异。结果 1每日补铁组及间歇性补铁组患者在身高及体重等生长发育指标增加值对比上无统计学意义(P>0.05);但两组患者各生长发育指标增加值均显著高于对照组(P<0.05);2每日补铁组和间歇性补铁组SF、Hb增加值均显著高于对照组,s Tf R、FER降低值则显著降低于对照组(P<0.05);每日补铁组除SF增加值显著高于间歇性补铁组(P<0.50)外,其余营养指标增加/降低值组间对比均无统计学意义(P>0.05)。结论对学龄前儿童予以补铁措施,可有效改善其营养状态,对促进体格生长发育有利。学龄前儿童家属可根据幼儿机体铁元素缺乏程度选择最佳补铁方式以改善其铁营养状况。
Objective To investigate the effects of daily iron supplementation and intermittent iron supplementation on the growth and development and iron nutrition status of preschool children. Methods A total of 424 preschool children aged 3 ~ 6 years old from a trustee institution of child care were enrolled in this study. Patients were divided into two groups: daily iron supplementation group (n = 133), intermittent iron supplementation group (n = 133) and control group (n = 158) three groups. During the 3-month follow-up, compared with the growth and developmental values of body weight, height, serum ferritin (SF), hemoglobin (Hb), serum transferrin receptor (sTfR) , Erythrocyte free protoporphyrin (FER) and other nutritional indicators increase / decrease the difference. Results 1 There was no significant difference between the daily iron supplementation group and the intermittent iron supplementation group in the growth and development indexes such as body height and body weight (P> 0.05). However, the growth and development indexes in both groups were significantly higher than those in the control group (P <0.05) .2 The increase of SF and Hb in the iron supplementation group and the intermittent iron supplementation group were significantly higher than those in the control group (P <0.05) (P> 0.05). There was no significant difference between the two groups (P> 0.05) except for the increase of SF in daily iron supplementation group compared with intermittent iron supplementation group (P <0.50). Conclusion Pre-school children with iron supplementation measures can effectively improve their nutritional status, to promote physical growth and development benefits. Family members of preschoolers can choose the best way of iron supplementation to improve their iron nutrition status according to the level of iron deficiency in children.
