目的观察CO2激光联合5-氨基酮戊酸光动力疗法治疗鲍温样丘疹病的临床疗效及安全性。方法将52例鲍温样丘疹病患者随机分为2组。治疗组32例为CO2激光联合光动力,患者先用CO2激光去除病变组织,再采用5-氨基酮戊酸光动力对患处行照光治疗,每周1次,连续3次;对照组20例为单纯CO2激光治疗组。治疗后第1周进行疗效评价,在治疗后3个月内进行随访以评价复发率。结果 2组患者治疗1周后评估皮损均完全清除。随访3个月后治疗组患者复发2例,复发率6.25%,对照组患者复发7例,复发率35.00%。2组之间复发率差异有统计学意义(χ2=4.81,P<0.05)。结论 CO2激光联合5-氨基酮戊酸光动力治疗鲍温样丘疹病是一种安全有效,能降低复发率的治疗方案。
Objective To observe the clinical efficacy and safety of CO2 laser combined with 5-aminolevulinic acid photodynamic therapy in the treatment of Bowenoid papulosis. Methods 52 cases of Bowenoid papulosis were randomly divided into two groups. In the treatment group, 32 cases were treated with CO2 laser combined with photodynamic therapy. The patients were firstly treated with CO2 laser to remove the lesion tissue, and then irradiated with 5-aminolevulinic acid photodynamic therapy on the affected area once a week for 3 times. In the control group, 20 cases were CO2 laser treatment group. The first week after treatment efficacy evaluation, follow-up within 3 months after treatment to evaluate the recurrence rate. Results The lesions were completely eliminated in both groups after 1 week of treatment. After 3 months of follow-up, patients in the treatment group relapsed in 2 cases, the recurrence rate was 6.25%. In the control group, 7 patients relapsed, the recurrence rate was 35.00%. There was significant difference in recurrence rate between the two groups (χ2 = 4.81, P <0.05). Conclusion CO2 laser combined with 5-aminolevulinic acid photodynamic therapy for Bowenoid papulosis is a safe and effective treatment that can reduce the recurrence rate.
