目的探讨65岁及以上老年人群糖化血红蛋白(Hb A1c)水平与血清糖类抗原19-9(CA19-9)、癌胚抗原(CEA)等肿瘤标记物的关系。方法对航天中心医院2012年参加健康体检的730名≥65岁老年人进行CA19-9、CEA、Hb A1c和血脂检测。结果 730名老年人中,Hb A1c偏高占10.14%。单因素分析显示,Hb A1c正常组(Hb A1c<7.0%)与偏高组(Hb A1c≥7.0%)在高甘油三酯血症、低高密度脂蛋白胆固醇血症、CA19-9和CEA等指标间差异均有统计学意义(P<0.05);多因素Logistic回归分析显示,高甘油三酯血症、低高密度脂蛋白胆固醇血症、CEA升高和CA19-9升高是血清Hb A1c升高的影响因素,其OR值(90%CI)分别为1.663(1.077~2.569)、2.202(1.296~3.741)、3.903(1.169~13.029)和5.232(2.532~10.809)。结论65岁及以上老年人群血清Hb A1c水平与CA19-9、CEA存在关联,血糖控制不佳的老年人群建议关注消化系统血清肿瘤标记物水平。
Objective To investigate the relationship between Hb A1c levels and tumor markers of serum carbohydrate antigen 19-9 (CA19-9) and carcinoembryonic antigen (CEA) in the elderly population aged 65 years and above. Methods A total of 730 elderly patients aged 65 or older who participated in the physical examination in the Space Central Hospital in 2012 were tested for CA19-9, CEA, Hb A1c and blood lipids. Results Of 730 elderly people, Hb A1c was 10.14%. Univariate analysis showed that in Hb A1c normal group (Hb A1c <7.0%) and in the high group (Hb A1c≥7.0%) in hypertriglyceridemia, low-density lipoprotein cholesterol, CA19-9 and CEA Logistic regression analysis showed that hypertriglyceridemia, low-density lipoprotein cholesterol, CEA increased and CA19-9 increased serum Hb A1c (90% CI) were 1.663 (1.077-2.569), 2.202 (1.296-3.741), 3.903 (1.169-13.029) and 5.232 (2.532-10.809), respectively. Conclusion Serum Hb A1c levels in the elderly population aged 65 years and older are associated with CA19-9 and CEA. The elderly with poor glycemic control are advised to pay attention to the levels of serum tumor markers in the digestive system.
