目的:观察术前干眼治疗伴睑板腺功能障碍(MGD)的白内障患者对术后干眼的影响。方法:前瞻性病例对照研究。将遵义医科大学第五附属(珠海)医院2019年1月至12月伴有MGD性干眼的白内障手术100例(118眼)患者随机分为两组。临床试验组45例(54例)，手术前3天进行个性化系统的干眼治疗，包括滴左氧氟沙星滴眼液，用庆大霉素注射液及玻璃酸钠滴眼液进行双眼热敷、超声雾化及睑板腺按摩的系统干眼治疗；对照组55例(64眼)仅滴左氧氟沙星滴眼液及玻璃酸钠滴眼液。两组均行晶状体超声乳化人工晶状体植入术。分别于术前及术后1周记录患者的干眼主觉症状和客观指标，包括睑板腺排出功能、分泌物、泪膜破裂时间(BUT)和泪液分泌功能试验(SIt)，以评价两组术后干眼的改善情况。结果:试验组术后干眼的主觉症状及客观指标的改善程度均优于对照组(n P<0.05)。n 结论:对伴有MGD性干眼白内障患者术前个性化系统的干眼治疗可以明显改善术后的干眼状况。“,”Objective:To observe the effect of preoperative dry eye treatment on postoperative dry eye in patients with cataract accompanied with meibomian gland dysfunction (MGD).Methods:A prospective case-control study. A total of 118 eyes of 100 patients who underwent surgery for cataract accompanied with MGD-related dry eye from Jan. to Dec. 2019 in the Fifth Affiliated Hospital of Zunyi Medical University were selected. The patients were randomly divided into two groups. At 3 days before surgery, 54 eyes of 45 cases in the clinical trial group received individualized and systematic dry eye treatment, including levofloxacin eye drops, gentamicin injection, sodium hyaluronate eye drops, hot compress of both eyes, ultrasonic atomization and meibomian gland massage. While 64 eyes of 55 cases in the control group was only treated with levofloxacin eye drops and sodium hyaluronate eye drops. Phacoemulsification and intraocular lens implantation were performed in both groups. The subjective symptoms and objective indexes of dry eye were recorded before and 1 week after surgery, including meibomian gland function, secretions, tear break-up time (BUT) and Schirmer I test (SIt), to evaluate the improvement in postoperative dry eye of the two groups.Results:The improvement in subjective symptoms and objective indexes of postoperative dry eye in the trail group were better than that in the control group (n P<0.05).n Conclusion:For patients with cataract accompanied with MGD, individualized and systematic preoperative dry eye treatment can significantly improve postoperative dry eye.
