血管平滑肌瘤多发于皮下组织,与皮肤粘连,直径一般不超过1cm。来源于上纵隔的血管平滑肌瘤罕见,现报告1例。患者,女,52岁。1989年9月16日入院,入院前5个月感右上胸部胀痛,无发热、咳嗽、咳痰等表现,近10余天出现右上肢酸软无力。查体:表浅淋巴结无肿大,头颈无异常,右前胸第2、3肋间局限性隆起略高于左侧,心肺无异常,肝脾不肿大。心肺功能正常,肝功能及血沉正常。正位胸片示:肺尖近纵隔处见4×3.5cm圆形密度增高、均匀阴影、边界清、肋骨无破坏。侧位片示:肿块阴影位于上后纵隔、与脊柱部分重叠。术前诊断:1(?)
Vascular leiomyomas are mostly found in subcutaneous tissue and adhere to the skin, and their diameter usually does not exceed 1 cm. Vascular leiomyomas from the upper mediastinum are rare and are reported in 1 case. The patient, female, is 52 years old. She was admitted to hospital on September 16, 1989, and felt pain in the right upper and upper chest five months before admission. There was no fever, cough, and cough. The right upper extremity was soft and weak for more than 10 days. Physical examination: Superficial lymph nodes were not swollen, head and neck were not abnormal, and the 2nd and 3rd intercostal limitations of the right anterior bulge were slightly higher than that of the left side. There was no abnormality in the heart and lungs, and the liver and spleen were not swollen. Cardiopulmonary function is normal, liver function and erythrocyte sedimentation rate are normal. Orthographic chest X-ray shows: the apex of the lung near the mediastinum see 4 × 3.5cm circular density increased, uniform shadows, clear borders, no damage to the ribs. The lateral view shows that the shadow of the mass lies in the upper posterior mediastinum and partially overlaps the spine. Preoperative diagnosis: 1 (?)
