摘 要:目的 觀察利多卡因气雾剂用于电子支气管镜检查的临床麻醉效果和安全性。方法 选取2019年3月~2020年6月安徽省胸科医院内镜室280例电子支气管镜检查患者,按照随机数表法分为两组,观察组(利多卡因气雾剂组)和对照组(利多卡因雾化吸入组)各140例。观察插管前1 min(T1)、插管即刻(T2)、插管后3 min(T3)各组的收缩压(SBP)、舒张压(DBP)、呼吸频率(RR)、心率(HR)变化情况,并记录麻醉效果、医生满意度和患者自我感受,评价两种麻醉方法的麻醉效果和安全性。结果 在T1基础上,观察组T2、T3的SBP、DBP、RR和HR上升量均明显低于对照组,差异均有统计学意义(P<0.05)。观察组麻醉效果优良率、医生评价优良率、患者满意度均高于对照组,差异有统计学意义(P<0.05)。结论 利多卡因气雾剂用于电子支气管镜检查表面麻醉,用量少,操作简单,麻醉效果好,患者血流动力学平稳,医生和患者满意度高,安全性高,临床可作为常规麻醉方法。

Clinical application of lidocaine aerosol in bronchoscope examination

Abstract: Objective To annalyze the clinical anesthesia effect and safety of lidocaine aerosol in the application of bronchscopy. Methods A total of 280 patients receiving flexible bronchoscopy in Chest Hospital of Anhui Province during the period from March 2019 to January 2020 were randomly divided into two groups according to the random number table method, including observation group and control group, 140 patients in each group. The observation group was treated with lidocaine aerosol, and the control group was treated with lidocaine inhalation. The changes in systolic blood pressure(SBP),diastolic blood pressure(DBP),respiratory rate(RR) and heart rate(HR) in different points of time, 1 min before tubation operation, the moment and 3 min after tubation operation. What’s more, the effect of local anesthesia, the satisfaction of both of the doctors and patients were observed during and after operation. Results A total of 280 patients completed the study. Comparing with the time points of 1 min before intubation(T1) , the increased levels of SBP, DBP, RR and HR at the time points of intubation(T2) and 3 min after extubation(T3) in the observation group were significantly less than those in the control group(P<0.05). The effect of local anesthesia, the satisfaction of the doctors and the patients with the treatment of lidocaine aerosol were significantly higher than the treatment of lidocaine inhalation(P<0.05). Conclusion Lidocaine aerosol for bronchoscopy anesthesia is a safe, easy and effective method, which is also accepted by both of the doctors and patients.
