目的对甘肃省静宁县1996-2013年孕产妇死亡监测结果进行分析,了解孕产妇死亡原因,寻找影响孕产妇死亡率下降的相关因素,为相关部门制定干预措施提供科学依据。方法采用回顾性分析的方法,按照国家《孕产妇死亡监测方案》,对逐级上报的孕产妇死亡卡及评审资料进行分析。结果 1996-2013年静宁县活产数93 793例,孕产妇死亡74例,死亡率为78.90/10万,孕产妇死亡率整体呈下降趋势。74例孕产妇前4位死因顺序依次为产科出血34例(45.95%),妊娠高血压疾病15例(20.27%),妊娠合并症10例(13.51%),羊水栓塞9例(12.16%)。死亡孕产妇中年龄≥30岁的占55.41%,初中以下文化程度占94.60%,居住地在农村占94.59%,100%(74/74)的死亡孕产妇家庭经济收入低于2000元。29.73%的死亡孕产妇未接受产前检查服务,54.05%(40/74)的死亡孕产妇产前检查次数<5次。结论 1996-2013年静宁县孕产妇死亡率显著下降,因此,继续开展农村孕产妇补助项目、降消项目,加强产前、产时保健,能进一步降低孕产妇死亡率。
Objective To analyze the maternal mortality monitoring results from 1996 to 2013 in Jingning County, Gansu Province, and to understand the causes of maternal death and find the factors that affect the decline of maternal mortality rate, so as to provide a scientific basis for the relevant departments to formulate intervention measures. Methods The retrospective analysis method was used to analyze the maternal death card and the reviewing data reported step by step according to the national “maternal death monitoring program”. Results From 1996 to 2013, 93 793 live births and 74 maternal deaths were reported in Jingning County, with a mortality rate of 78.90 / 100 000. The overall maternal mortality rate showed a downward trend. The order of the causes of death among the 74 pregnant women was 34 cases (45.95%), 15 cases (20.27%) of pregnancy-induced hypertension, 10 cases of pregnancy complications (13.51%) and amniotic fluid embolism in 9 cases (12.16%). 55.41% of the pregnant women aged 30 or older, 94.60% of them have education below junior high school, 94.59% of them live in rural areas, and 100% (74/74) of the deaths of pregnant women have economic income of less than 2,000 yuan. 29.73% of the deceased pregnant women did not receive prenatal care and 54.05% (40/74) of the pregnant women lost their mothers the number of prenatal care was less than 5 times. Conclusions From 1996 to 2013, the maternal mortality rate in Jingning County dropped significantly. Therefore, continuing to carry out the rural maternal subsidy project, eliminating the project and strengthening prenatal and postnatal care will further reduce the maternal mortality rate.
