目的通过调查张掖市部分不同性别、不同年龄人群的HCV感染现状及血清中的病毒载量,以研究该地区HCV感染及病毒复制状况。方法对2008年6月至2009年6月于张掖市人民医院住院和门诊部就诊的患者13743例(男8945例,女4798例)进行传染病三项(HBsAg、抗-HCV、抗-HIV)筛查,查出抗-HCV阳性者,再进一步行HCVRNA检测。抗-HCV血清学检查采用酶联免疫法,HCVRNA检测采用实时荧光定量PCR法。结果抗-HCV阳性率为3.74%,女性抗-HCV阳性率4.46%,高于男性的3.35%(χ2=10.63,P<0.01)。抗-HCV阳性者中,HCVRNA阳性率为45.53%,HCVRNA男性阳性率为42.00%,女性为50.47%,男女阳性率比较,差异无统计学意义(χ2=3.61,P>0.05)。抗-HCV阳性者5个年龄组构成比由高到低依次为C组42.02%、D组21.40%、B组18.68%、A组9.34%、E组8.56%。5个不同年龄组间HCVRNA阳性率差异无统计学意义(χ2=6.168,P>0.05)。结论该地区HCV的发病有下降趋势,已感染人群中女性多于男性。HCVRNA阳性率不同性别及不同年龄组间差异不显著,但监测HCVRNA可指导临床用药及观察疗效。
Objective To investigate the prevalence of HCV infection and serum viral load in some population of different sexes and ages in Zhangye city in order to study the status of HCV infection and virus replication in this area. Methods A total of 13 743 patients (8945 males and 4798 females) were treated in the inpatient and outpatient department of Zhangye People’s Hospital from June 2008 to June 2009 for three infectious diseases (HBsAg, anti-HCV and anti-HIV) Screening, detection of anti-HCV-positive, and then further HCV RNA detection. Anti-HCV serology using enzyme-linked immunosorbent assay, HCV RNA detection using real-time fluorescence quantitative PCR method. Results The positive rate of anti-HCV was 3.74%. The positive rate of anti-HCV in female was 4.46%, higher than that of males (χ2 = 10.63, P <0.01). The positive rate of HCVRNA was 45.53%, the positive rate of HCVRNA was 42.00%, and the rate of HCVRNA was 50.47%. There was no significant difference between male and female (χ2 = 3.61, P> 0.05). The anti-HCV positive 5 age groups were 42.02% in C group, 21.40% in D group, 18.68% in B group, 9.34% in A group and 8.56% in E group. There was no significant difference in the positive rates of HCV RNA among 5 different age groups (χ2 = 6.168, P> 0.05). Conclusion The incidence of HCV in this area has a downward trend, with more infected women than men. The positive rate of HCVRNA was not significantly different between different genders and different age groups. However, the monitoring of HCVRNA could guide the clinical medication and observe the curative effect.
