目的 :利用利普液基细胞学(LPT)技术及免疫细胞化学技术,评价半乳糖凝集素-3(Galectin-3)和甲状腺过氧化物酶(thyroid peroxidase,TPO)在甲状腺良恶性结节中的诊断价值。方法 :甲状腺细针穿刺标本80例LPT技术制细胞涂片,用免疫细胞化学方法检测穿刺标本中Galectin-3与TPO的表达情况,并与甲状腺术后组织病理相比较,评价Galectin-3和TPO在甲状腺良恶性病变中的表达率。结果:术前行细针穿刺细胞学(fine needle aspiration cytology,FNAC)检查的80例患者术后病理确诊为甲状腺癌51例(其中甲状腺乳头状癌49例、甲状腺滤泡状腺癌2例),良性病变29例(含慢性淋巴细胞性甲状腺炎2例、滤泡性腺瘤7例、结节性甲状腺肿20例)。免疫组化检测Galectin-3在甲状腺乳头状癌和滤泡性腺癌中明显高表达,分别为93.8%和100%,总阳性率为94%(47/51);TPO在慢性淋巴细胞性甲状腺、结节性甲状腺肿和滤泡性腺瘤中明显高表达,阳性表达率分别为100%、100%和85.7%,总阳性表达率为96.7%(28/29)。结论:液基细胞学技术与免疫细胞化学分析相结合在甲状腺结节FNAC标本诊断中有重要的临床意义,细胞分子标志物Galectin-3和TPO可用于甲状腺良恶性肿瘤的鉴别诊断,提高了甲状腺癌的术前诊断率,值得在临床细胞病理学诊断中推广应用。
OBJECTIVE: To evaluate the role of Galectin-3 and thyroid peroxidase (TPO) in benign and malignant thyroid nodules using Lipofectamine (LPT) and immunocytochemistry The diagnostic value. Methods: Thirty thyroid needle aspirates were used to make cell smears by using LPT technique. The expressions of Galectin-3 and TPO in the puncture specimens were detected by immunocytochemistry. Results: Preoperative diagnosis of 80 cases of fine needle aspiration cytology (FNAC) pathologically confirmed 51 cases of thyroid cancer (thyroid papillary carcinoma in 49 cases, thyroid follicular adenocarcinoma in 2 cases) , Benign lesions in 29 cases (including 2 cases of chronic lymphocytic thyroiditis, follicular adenoma in 7 cases, nodular goiter in 20 cases). Immunohistochemical detection of Galectin-3 in papillary thyroid carcinoma and follicular adenocarcinoma was significantly higher expression, respectively 93.8% and 100%, the total positive rate was 94% (47/51); TPO in chronic lymphocytic thyroid, Nodular goiter and follicular adenoma. The positive rate was 100%, 100% and 85.7%, respectively. The total positive expression rate was 96.7% (28/29). Conclusion: The combination of liquid-based cytology and immunocytochemistry in the diagnosis of thyroid nodules FNAC specimens has important clinical significance.
