目的观察不同剂量地佐辛用于小儿术后镇痛的临床效果,并就其安全性予以评价。方法选取医院收治的小儿手术治疗患儿45例,根据术后镇痛药物地佐辛使用剂量的不同分为A、B、C组,每组15例。3组镇痛药物均用地佐辛、盐酸昂丹司琼加入0.9%氯化钠注射液,只在地佐辛剂量上存在差别(A组剂量0.6 mg/kg,B组剂量0.8 mg/kg,C组剂量1.0 mg/kg)。观察药物镇痛不良反应,记录3组患儿在手术后4、12、24、48 h各个时间点的心率(HR)、平均动脉压(MAP)、血氧饱和度(Sp O2)、呼吸频率(RR)等变化情况;针对最终的镇痛效果予以评分即面部疼痛表情量表(FPS-R)、CHEOPS疼痛评分。结果 3组患儿在分别予以不同剂量地佐辛镇痛药物后,A组不良反应发生率为20.00%,B组不良反应发生率为26.67%,C组不良反应发生率为40.00%,A组、B组与C组比较,差异有统计学意义(P<0.05)。在FPS-R/CHEOPS评分上,A组与B组、C组比较,差异有统计学意义(P<0.05)。3组HR、MAP、Sp O2、RR术前、术后各指标变化均较平稳,且A组各指标较优良,患儿各项生命体征相对而言最平稳,临床安全性较好。结论 0.6 mg/kg+盐酸昂丹司琼0.1 mg/kg+0.9%氯化钠注射液方案,可作为患儿腹部手术静脉自控镇痛时最佳选择,可有效降低不良反应发生率。
Objective To observe the clinical effect of dezocine in different doses on postoperative analgesia in pediatric patients and evaluate its safety. Methods Forty-five children with pediatric surgery were selected and divided into A, B and C groups according to the dose of dezocine used in the postoperative analgesia. Each group included 15 cases. 3 groups of analgesics were Zozin, Ondansetron hydrochloride 0.9% sodium chloride injection, only in the dezocine dosage differences (A dose of 0.6 mg / kg, B dose of 0.8 mg / kg, Group C dose 1.0 mg / kg). The analgesic adverse reaction was observed. The heart rate (HR), mean arterial pressure (MAP), oxygen saturation (Sp O2) and respiratory rate were recorded at 4, 12, 24 and 48 h after operation in all three groups (RR) and other changes; for the final analgesic effect to be scored Facial Pain Rating Scale (FPS-R), CHEOPS pain score. Results The incidence of adverse reactions in group A was 20.00%, the incidence of adverse reactions in group B was 26.67% and the rate of adverse reactions in group C was 40.00% after being treated with dezocine in different doses respectively. Group A , There was significant difference between group B and group C (P <0.05). The FPS-R / CHEOPS score, A group and B group, C group, the difference was statistically significant (P <0.05). The changes of HR, MAP, Sp O2 and RR in 3 groups were stable before and after surgery, and the indexes in group A were superior. The vital signs of children were the most stable and the clinical safety was better. Conclusion 0.6 mg / kg of ondansetron hydrochloride 0.1 mg / kg + 0.9% sodium chloride injection solution can be used as the best choice for controlled abdominal pain in patients with abdominal surgery, which can effectively reduce the incidence of adverse reactions.
