目的观察参附注射液对家兔心肺复苏后脑海马组织超微结构的影响,评价其对复苏后脑组织的保护作用。方法随机将家兔分为正常复苏组、假手术组、参附注射液组,正常复苏组、参附注射液组建立心肺复苏模型,自主循环恢复后每隔30min分别缓慢静注生理盐水、参附注射液1次,连续3次;假手术模型组不建立心肺复苏模型。均在8h取脑海马组织,电镜观察脑组织超微结构改变。结果正常复苏组脑超微结构广泛水肿、结构破坏,胶质细胞核坏死、空泡改变,胞浆内无结构、稀少、无细胞器,髓鞘、轴浆萎缩、空泡。假手术模型组超微结构完好,胶质细胞核、胞浆完整、胞质水肿不明显,髓鞘完整,轴浆无萎缩,血管内皮肿胀、管腔变狭窄。参附注射组:脑超微结构尚完好,胶质细胞核正常、胞浆完整、胞质突起肿胀,部分胶质细胞内细胞器少、空泡破坏(变性),见少量线粒体;髓鞘完整,血管内皮细胞完整。结论心肺复苏时应用参附注射液可以减轻脑海马组织超微结构损害,对复苏后脑组织有一定的保护作用。
Objective To observe the effect of Shenfu injection on the ultrastructure of hippocampus in rabbits after cardiopulmonary resuscitation and evaluate its protective effect on brain tissue after resuscitation. Methods The rabbits were randomly divided into normal resuscitation group, sham operation group, and Shenfu injection group. The cardiopulmonary resuscitation model was established in the normal resuscitation group and Shenfu injection group. The normal saline and ginseng were intravenously injected every 30 minutes after the recovery of spontaneous circulation. Injections were given once for 3 consecutive times; the sham-operated model group did not establish a cardiopulmonary resuscitation model. Brain hippocampal tissue was taken at 8h and electron microscopy was used to observe the changes of brain ultrastructure. RESULTS: In the normal resuscitation group, the ultrastructure of the brain was extensively edema, structural damage, necrosis of glial cells and vacuolization, no cytoplasmic structure, rare, no organelles, myelin sheath, axonal atrophy, vacuolization. In the sham-operated model group, the ultrastructure was intact, the glial cell nucleus, cytoplasm was intact, the cytoplasmic edema was not obvious, the myelin sheath was intact, the axonole was not atrophic, the vascular endothelium was swollen, and the lumen became narrow. Shenfu injection group: brain ultrastructure is still intact, glial nuclei are normal, cytoplasm is complete, cytoplasmic processes are swollen, part of the glial cells have less organelles, vacuole destruction (denaturation), see a small amount of mitochondria; myelin sheath intact, blood vessels Endothelial cells are intact. Conclusion Shenfu injection can reduce the ultrastructural damage of hippocampus tissue in cardiopulmonary resuscitation and protect brain tissue after resuscitation.
