目的探讨老年高血压患者脉压(PP)及脉压指数(PPI)与颈动脉粥样硬化的关系。方法通过彩色多普勒超声检查98例老年高血压患者颈动脉粥样硬化斑块及最大颈总动脉内中膜厚度(IMTmax),按颈动脉超声分为正常组和颈动脉斑块组,行24h动态血压监测,计算出平均脉压及脉压指数,观察两组脉压及脉压指数的差异以及IMTmax与脉压及脉压指数的相关性。结果颈动脉斑块组PP及PPI均较颈动脉正常组增高(P<0.05)。IMTmax与PP及PPI密切相关(r分别为0.357和0.412,P均<0.01)。结论脉压及脉压指数与颈动脉粥样硬化密切相关,且脉压指数在一定程度上较脉压有更大优势。
Objective To investigate the relationship between pulse pressure (PP) and pulse pressure index (PPI) and carotid atherosclerosis in elderly patients with hypertension. Methods The carotid atherosclerosis plaque and the maximal common carotid artery intima-media thickness (IMTmax) in 98 elderly patients with hypertension were examined by color Doppler ultrasonography and divided into normal group and carotid artery plaque group according to carotid ultrasonography. 24h ambulatory blood pressure monitoring to calculate the mean pulse pressure and pulse pressure index, pulse pressure and pulse pressure index differences were observed between the two groups and IMTmax and pulse pressure and pulse pressure index. Results The carotid plaque group PP and PPI were higher than the normal carotid artery group (P <0.05). IMTmax was closely related to PP and PPI (r = 0.357 and 0.412, P <0.01 respectively). Conclusion Pulse pressure and pulse pressure index are closely related to carotid atherosclerosis, and pulse pressure index has a greater advantage than pulse pressure to a certain extent.
