目的观察盆底器官脱垂患者阴道前壁组织中中性粒细胞弹性蛋白酶(NE)及弹性蛋白酶抑制剂(Elafin)的表达水平。探讨其与盆底器官脱垂的发生关系。方法自2012年4月1日-2013年12月31日该科收治的60例子宫脱垂患者(绝经前26例,绝经后34例),采用real time PCR及Western blot法检测60例子宫脱垂患者(观察组)阴道前壁组织中NE及Elafin的m RNA和蛋白的表达水平,选择同期因妇科良性疾病、无盆底功能障碍性疾病而行子宫全切除术的患者40例(对照组)做比较。结果观察组患者阴道前壁组织中NE的m RNA表达水平为(1.53±0.26),NE蛋白表达水平为(2.08±0.48);Elafin的m RNA表达水平为(1.36±0.25),Elafin蛋白表达水平为(0.84±0.23)。对照组中NE的m RNA表达水平为(0.63±0.22),NE蛋白表达水平为(1.29±0.35);Elafin的m RNA表达水平为(0.57±0.22),Elafin蛋白表达水平为(0.30±0.16)。观察组与对照组比较,差异均有统计学意义(P<0.01),观察组高于对照组。观察组与对照组患者阴道前壁组织中NE及Elafin的m RNA和蛋白表达水平在绝经前、后差异均无统计学意义(P>0.05)。NE与Elafin的表达量呈直线正相关关系(r=0.931、0.782,P<0.05)。结论 NE及Elafin表达的异常可能与盆底器官脱垂的发病有关。
Objective To investigate the expression of neutrophil elastase (NE) and elastase inhibitor (Elafin) in the anterior vaginal tissue of patients with pelvic organ prolapse. Explore its relationship with pelvic organ prolapse. Methods From April 1, 2012 to December 31, 2013, 60 patients with uterine prolapse (26 premenopausal women and 34 postmenopausal women) were enrolled in this study. Real time PCR and Western blot were used to detect 60 cases of uterine prolapse 40 patients (control group) undergoing hysterectomy due to benign gynecological diseases and no pelvic floor dysfunction during the same period were selected as control group )compare with. Results In the observation group, the m RNA expression level of NE in the anterior vaginal tissue was (1.53 ± 0.26) and the NE protein expression level was (2.08 ± 0.48); the m RNA expression level in Elafin was (1.36 ± 0.25) and Elafin protein expression level (0.84 ± 0.23). The level of m RNA in NE was (0.63 ± 0.22) and (1.29 ± 0.35) in NE and the expression of Elafin was (0.57 ± 0.22) and (0.30 ± 0.16) . The difference between the observation group and the control group was statistically significant (P <0.01), the observation group was higher than the control group. The mRNA and protein levels of NE and Elafin in the anterior vaginal tissues of the observation group and the control group were not significantly different before and after menopause (P> 0.05). There was a linear positive correlation between NE and Elafin expression (r = 0.931,0.782, P <0.05). Conclusion The abnormal expression of NE and Elafin may be related to the pathogenesis of pelvic organ prolapse.
