目的研究代谢手术对2型糖尿病患者认知功能的影响。方法选取2015年4月至2016年6月第三军医大学大坪医院住院的65例2型糖尿病患者,其中行代谢手术的患者30例,未行手术治疗的患者35例,分别完成体检、实验室检查、认知功能量表评估,并对两组患者一般资料及认知功能进行统计分析。结果在校正两组患者一般资料之前,在总体认知功能评价、记忆力与执行力、语言流畅性、视觉空间能力方面,手术组优于非手术组(P<0.05),在注意力与定向力方面,两组差异无统计学意义(P>0.05);在校正两组患者一般资料之后,在记忆力、执行力、语言流畅性方面,手术组优于非手术组(P<0.05);在总体认知功能评价、注意力与定向力、视觉空间能力方面,两组差异无统计学意义(P>0.05)。两组患者的认知功能与体重指数、血压、血糖、血脂及高敏C反应蛋白无相关性。结论代谢手术能改善糖尿病患者认知功能,且其机制可能独立于糖尿病患者体重、血压、糖脂代谢的改善。
Objective To study the effect of metabolic surgery on cognitive function in patients with type 2 diabetes mellitus. Methods From April 2015 to June 2016, 65 patients with type 2 diabetes were enrolled in Daping Hospital of the Third Military Medical University. Among them, 30 patients underwent metabolic surgery and 35 patients underwent surgical treatment. The physical examination, laboratory Examination, assessment of cognitive function scale, and the two groups of patients with general information and cognitive function for statistical analysis. Results Before the general information of two groups of patients was corrected, the surgical group was better than the non-surgical group in terms of overall cognitive function evaluation, memory and executive ability, language fluency and visual spatial ability (P <0.05) There was no significant difference between the two groups (P> 0.05). After adjusting the general information of two groups of patients, the operation group was superior to the non-operation group in memory, executive ability and language flu (P <0.05) There was no significant difference between the two groups in cognitive function evaluation, attention and orientation, visual spatial abilities (P> 0.05). There was no correlation between cognitive function and body mass index, blood pressure, blood glucose, blood lipids and high-sensitivity C-reactive protein in both groups. Conclusion Metabolic surgery can improve the cognitive function of diabetic patients, and its mechanism may be independent of body weight, blood pressure and glucose and lipid metabolism in diabetic patients.
