血清免疫球蛋白游离轻链(FLC)κ和λ测定是浆细胞病诊断和疗效分析及预后判断的重要工具。FLC分析与血清蛋白电泳和免疫固定电泳联合可明显提高浆细胞病诊断的敏感性,并在除轻链淀粉样变性外的其它疾病中替代24h尿液检查。基础FLC可预测各种浆细胞病的预后。对轻链淀粉样变性、寡分泌型和非分泌型骨髓瘤,可通过FLC分析进行定量监测。对轻链淀粉样变性,连续FLC检测优于蛋白电泳和免疫固定电泳。
Serum immunoglobulin free light chain (FLC) κ and λ determination of plasma cell disease diagnosis and efficacy analysis and prognosis of an important tool. FLC analysis in combination with serum protein electrophoresis and immunostaining gel electrophoresis can significantly improve the diagnostic sensitivity of plasmapheresis and replace the 24 h urinalysis in other diseases other than light chain amyloidosis. Fundamental FLC predicts the prognosis of various plasma cell disorders. For light chain amyloidosis, oligoclonal and non-secreting myeloma, quantitative monitoring can be performed by FLC analysis. For light chain amyloidosis, continuous FLC detection is superior to protein electrophoresis and immunofixation electrophoresis.
