目的探讨MRI在儿童神经管发育畸形筛查诊断中的应用价值。方法选取产前接受神经系统功能检查的546例儿童作为研究对象,分别行超声和MRI检查;比较不同检测方法下神经管畸形儿童判定结果、与手术结果符合率以及各自诊断信息。结果本组546例共检出59例儿童存在神经管发育畸形,且均经手术证实。其中二种方法检出结果一致52例,7例不符,以手术结果为标准,MRI检查符合率98.72%,诊断特异性为98.99%,敏感性为95.92%,漏诊率为4.08%,误诊率为1.01%。59例畸形儿主要分布在新生儿~12岁,其中50例经手术缓解,9例死亡。结论 MRI在诊断儿童神经管发育畸形中具有重要的意义,其能清晰显示脊柱软组织区及脊髓栓系各组织情况,为确保下一代健康,儿童在接受常规超声检查的同时再接受MRI检查,以便深入了解神经管畸形程度、评估脂肪瘤范围,为临床麻醉及手术提供更为可靠的信息。
Objective To investigate the value of MRI in the diagnosis of neural tube defects in children. Methods A total of 546 children with prenatal neurological function tests were selected as subjects. Ultrasound and MRI were performed respectively. The results of children with neural tube defects under different detection methods were compared with those of the surgical findings and their diagnostic information. Results A total of 596 children in our group were found to have degenerative neural tube defects in 546 cases, all confirmed by surgery. The results of the two methods were consistent in 52 cases, 7 cases did not match, the results of the surgery as the standard, MRI examination was 98.72%, diagnostic specificity was 98.99%, the sensitivity was 95.92%, misdiagnosis rate was 4.08%, the misdiagnosis rate was 1.01%. 59 cases of deformities were mainly distributed in neonates ~ 12 years old, of which 50 cases were relieved by operation and 9 cases died. Conclusion MRI plays an important role in the diagnosis of neural tube defects in children. It can clearly show the tissues of the spinal soft tissue and the tethered cord. To ensure the health of the next generation, children underwent routine MRI and MRI examination at the same time In-depth understanding of the degree of neural tube defects, assessment of lipoma range, for clinical anesthesia and surgery to provide more reliable information.
