目的:通过了解某市上级医院与社区卫生服务机构的基本服务情况,分析社区首诊中存在的问题,为某市社区首诊制度的进一步完善提出相关建议。方法:采用文献法和问卷调查方法收集资料,通过重点调查与抽样调查相结合,对某市3家综合性医院、7家社区卫生服务机构及周边居民进行问卷调查。结果:社区卫生服务机构与上级医院的医务人员在学历和执业年限上存在差距;社区卫生服务机构的日常接诊量低于上级医院的接诊量;社区医疗卫生资源不足是阻碍社区首诊有效落实的主要问题。结论:通过提高社区医疗服务的质量,加大社区首诊的宣传力度,加强居民健康管理等措施破解社区首诊的障碍。
OBJECTIVE: To understand the basic services of the higher-level hospitals and community health service institutions in a certain city, analyze the problems existing in the first consultation of the community, and make relevant suggestions for the further improvement of the first consultation system in a certain city. Methods: The methods of literature and questionnaires were used to collect data. Through the combination of key surveys and sampling surveys, questionnaires were conducted on three general hospitals, seven community health service agencies and surrounding residents in a certain city. Results: There was a gap between academic qualifications and years of practice in the community health service institutions and the medical staffs in the higher-level hospitals. The daily consultation rate of the community health service institutions was lower than that of the higher-level hospitals. Insufficient community medical and health resources hindered the effective community first diagnosis The main problem of implementation. Conclusion: By improving the quality of community medical services, increasing the publicity of community first consultation, strengthening residents’ health management and other measures to solve the problems of community first consultation.
