在3~37.8周/度,12级空间频率范围内用激光视网膜-视皮质闪烁正弦光栅4Hz、16Hz两级时频对34例(65只眼)早期青光眼患者,18例(31只眼)中晚期青光眼患者及19例(37只眼)疑似青光眼患者作了时空视觉调制传递函数(spatiotemporal modulation transfer function,S-TMTF)测定,并对上述疑似组中16例进行了随访观察。结果表明:早期青光眼患者低频区可出现异常的S-TMTF值,中频区这种差异已趋普遍化,高频区则下降更显著。疑似组的S-TMTF下降可见于9.5周/度以上的诸多频位,而低频区未见差异存在。各青光眼组16Hz S-TMTF值低于4Hz相应的各频位值,其最大差值随病情的加重呈现由高频位向低频位偏移趋势。6个月的随访复查中,一例高眼压女性患者,出现青光眼早期视野改变的表现。结果提示:S-TMTF诊断早期青光眼有高度的敏感性,但筛选特异性的诊断频位点及共临界值尚有困难。
Laser glaucoma-visual cortex scintillation sinusoidal grating 4Hz and 16Hz two-stage time-frequency were used in 34 patients (65 eyes) with early glaucoma and in 18 eyes (31 eyes) in the spatial frequency range of 3 ~ 37.8 weeks / Sixteen patients in the suspected group were followed up for observation of spatiotemporal modulation transfer function (S-TMTF) in patients with advanced glaucoma and in 19 patients (37 eyes) with suspected glaucoma. The results showed that abnormal S-TMTF values may appear in the low-frequency area of patients with early glaucoma. The difference in the mid-frequency area has become generalized, while the decline in the high-frequency area is more significant. The S-TMTF decrease in the suspected group was found in many frequency bands above 9.5 weeks / degree, while no difference was found in the low-frequency region. The 16 Hz S-TMTF values of glaucoma group were lower than those of 4 Hz corresponding to each frequency value. The maximum difference showed a trend of shift from high frequency to low frequency as the condition progressed. In a 6-month follow-up review, a case of hypertensive intraocular pressure in women showed a change in their early visual field. The results suggest that S-TMTF is highly sensitive in the diagnosis of early glaucoma. However, it is difficult to screen specific diagnostic frequency sites and the total cut-off value.
