目的:探讨分析血压变异参数与进展性卒中的相关性。方法:选取2010年1月~2011年12月间我院神经内科收治的急性缺血性卒中患者229例作为研究对象,其中属于进展性卒中的患者有42例,属于非进展性卒中的患者有187例。我们对这229例患者入院7天内的血压值进行连续记录,血压变异参数采用血压均值和标准差来表示。比较进展性卒中患者与非进展性卒中患者在血压及血压变异参数等方面的差异,并将比较结果及其临床资料进行回顾性的分析。结果:两组患者收缩压和舒张压的均值与标准差之间的差异不显著(P>0.05),不具有统计学意义。两组患者收缩压的标准差在10~20mmHg之间的频数分布存在较为显著的差异(P=0.047),具有统计学意义。两组患者舒张压的标准差频数分布差异不显著(P>0.05),不具有统计学意义。两组患者收缩压的Sd(标准差)与Mean(平均数)均呈正相关性(r=0.310,P=0.046;r=0.319,P=0.000)。PIS组患者舒张压的Sd与Mean无相关性(r=0.081,P=0.609)。NPIS组患者舒张压的Sd与Mean呈正相关性(r=0.202,P=0.006)。结论:收缩压的变异与进展性卒中的发生发展密切相关。
Objective: To investigate the correlation between blood pressure variability parameters and stroke. Methods: A total of 229 patients with acute ischemic stroke admitted to Department of Neurology in our hospital from January 2010 to December 2011 were selected as the study subjects. Of them, 42 patients were classified as progressive stroke and those with non-progressive stroke were 187 cases. We recorded these 229 patients within 7 days of continuous blood pressure values recorded, blood pressure variability parameters using mean and standard deviation to represent. The differences of blood pressure and blood pressure variability parameters between patients with progressive stroke and those without progressing stroke were compared, and the results of the comparisons and clinical data were retrospectively analyzed. Results: There was no significant difference between the mean and standard deviation of systolic pressure and diastolic pressure in both groups (P> 0.05), which was not statistically significant. There was a significant difference (P = 0.047) in the frequency distribution of systolic blood pressure between two groups in the range of 10-20 mmHg, with statistical significance. There was no significant difference in the standard deviation frequency of diastolic blood pressure between the two groups (P> 0.05), which was not statistically significant. Systolic blood pressure (Sd) and Mean (mean) were positively correlated between the two groups (r = 0.310, P = 0.046; r = 0.319, P = 0.000). There was no correlation between Sd and Mean of PIS group (r = 0.081, P = 0.609). There was a positive correlation between Sd and Mean of diastolic blood pressure in patients with NPIS (r = 0.202, P = 0.006). Conclusion: The variation of systolic blood pressure is closely related to the occurrence and development of progressive stroke.
