目的通过测量中央角膜全层、上皮层和基质层厚度,前基质层、后基质层和内皮细胞密度,观察配戴角膜塑形镜对角膜的组织学影响。方法收集哈尔滨医科大学附属第一医院验配角膜塑形镜的患者34例(34眼),戴镜前、戴镜后1天、1周、1个月、3个月、半年、1年时均使用海德堡共聚焦激光角膜显微镜检查,记录中央角膜全层、上皮层和基质层的厚度,前基质层、后基质层细胞密度和内皮细胞密度。结果与戴镜前相比,中央角膜全层厚度减少,一个月后趋于稳定,中央角膜上皮层厚度降低,1个月后趋于稳定,基质层厚度降低但变化无统计学意义,前基质层细胞密度变化无意义,后基质层细胞密度降低,6个月后趋于稳定,内皮细胞密度变化无意义。结论长期佩戴角膜塑形镜对角膜组织影响小,并且其改变在一定时间趋于稳定。
Objective To observe the histological effects of wearing orthokeratology on the cornea by measuring the thickness of the central cornea, the thickness of epithelium and stroma, the density of anterior stroma, posterior stroma and endothelial cells. Methods 34 cases (34 eyes) with matched orthokeratology lens were collected from the First Affiliated Hospital of Harbin Medical University. Before wearing the mirror, 1 day, 1 week, 1 month, 3 months, 6 months and 1 year after wearing the mirror Heidelberg confocal laser photomicrographs were used to record the thickness of the central corneal whole layer, the epithelium and stroma, the density of the anterior stroma, the stroma and the endothelial cell. Results Compared with those before wearing glasses, the thickness of central corneal whole layer decreased, and stabilized after one month. The thickness of central corneal epithelial layer decreased. After one month, the corneal thickness tended to be stable and the thickness of stromal layer decreased but with no statistical significance. Layer cell density changes meaningless, post-cell layer cell density decreased, after 6 months tends to be stable, endothelial cell density changes meaningless. Conclusion Long-term orthokeratology wear orthokeratology has little effect on corneal tissue, and its change tends to be stable in a certain period of time.
