：
目的 分析老年脑出血患者发生肺部感染的细菌分布情况,并探究血清中炎性因子降钙素原(PCT)、C反应蛋白(CRP)对老年脑出血合并肺部感染的诊断价值.方法 选取2017年12月—2020年5月于天津市第一中心医院就诊的脑出血老年患者151例,将74例脑出血未合并肺部感染的老年患者作为单纯脑出血组,将77例脑出血合并肺部感染的老年患者作为脑出血合并肺部感染组,收集同期80例老年健康体检者作为健康对照组.分析脑出血合并肺部感染组病原菌的分布情况,比较各组血清PCT和CRP水平的差异,通过受试者工作曲线(ROC)

目的 分析老年脑出血患者发生肺部感染的细菌分布情况,并探究血清中炎性因子降钙素原(PCT)、C反应蛋白(CRP)对老年脑出血合并肺部感染的诊断价值.方法 选取2017年12月—2020年5月于天津市第一中心医院就诊的脑出血老年患者151例,将74例脑出血未合并肺部感染的老年患者作为单纯脑出血组,将77例脑出血合并肺部感染的老年患者作为脑出血合并肺部感染组,收集同期80例老年健康体检者作为健康对照组.分析脑出血合并肺部感染组病原菌的分布情况,比较各组血清PCT和CRP水平的差异,通过受试者工作曲线(ROC)分析PCT、CRP及其联合检测对老年脑出血合并肺部感染的检测效能.结果 在77例脑出血合并肺部感染组患者标本中共检出病原菌95株,以革兰阴性菌为主,占68.42％,其次是革兰阳性菌.脑出血合并肺部感染组的PCT和CRP水平均高于单纯脑出血组和健康对照组,差异均有统计学意义(P＜0.05).ROC曲线结果表明,PCT和CRP联合检测的曲线下面积(AUC)为0.982,高于PCT的0.856和CRP的0.777.结论 老年脑出血合并肺部感染的病原菌以革兰阴性菌为主,血清PCT、CRP对其具有重要的临床诊断价值,且联合检测的诊断价值高于单一指标.
