目的比较牙体牙髓疾病实施一次性根管或多次法根管治疗的临床效果及并发症发生情况。方法回顾性分析2014年10月至2016年10月收治的169例牙体牙髓疾病患者的临床资料。实施多次法根管治疗84例为对照组,给予一次性根管治疗85例为观察组。观察两组的临床效果以及并发症发生情况,并进行比较。结果观察组显效50例,有效29例,无效6例,总有效率92.94%;对照组显效47例,有效30例,无效7例,总有效率91.67%,两组总有效率比较差异无统计学意义(χ~2=0.097,P=0.756)。术后观察组平均疼痛消失时间较对照组明显减少[(2.02±0.31)d vs(2.18±0.21)d,t=3.92,P=0.00]。术后两组用药时间差异无统计学意义[(3.15±0.33)d vs(3.19±0.37)d,t=0.74,P=0.46]。观察组、对照组治疗后疼痛、牙龈肿胀、咬合问题等并发症发生率分别为5.88%、5.95%,差异无统计学意义(χ~2=0.00,P>0.05)。结论一次性根管治疗与多次法根管治疗牙体牙髓疾病均能够获得理想的治疗效果,均属于可行的治疗方案。但一次性根管治疗术后疼痛消失时间较早。
Objective To compare the clinical effects and complications of one-time or multiple root canal treatment of dental pulp disease. Methods The clinical data of 169 patients with dental pulp diseases who were treated from October 2014 to October 2016 were analyzed retrospectively. The implementation of multiple root canal treatment of 84 cases as the control group, given a one-time root canal treatment of 85 cases for the observation group. The clinical effects and complications of the two groups were observed and compared. Results In the observation group, 50 cases were markedly effective, 29 cases were effective, 6 cases were ineffective and the total effective rate was 92.94%. In the control group, 47 cases were markedly effective, 30 cases were effective, 7 cases were ineffective and the total effective rate was 91.67%. There was no statistical difference between the two groups Significance (χ ~ 2 = 0.097, P = 0.756). The mean duration of pain disappearance in the postoperative observation group was significantly lower than that in the control group [(2.02 ± 0.31) d vs (2.18 ± 0.21) d, t = 3.92, P = 0.00]. There was no significant difference in postoperative medication time between the two groups [(3.15 ± 0.33) d vs (3.19 ± 0.37) d, t = 0.74, P = 0.46). The complication rates of pain, gingival swelling and occlusion in the observation group and control group were 5.88% and 5.95%, respectively, with no significant difference (χ ~ 2 = 0.00, P> 0.05). Conclusion One-time root canal therapy and multiple root canal treatment of dental pulp disease are able to obtain the ideal therapeutic effect, are feasible treatment options. However, one-time root canal treatment of postoperative pain disappear earlier.
