目的研究育龄期女性原发性癫痫间患者卵巢动脉血流动力学和性激素水平的变化。方法癫痫间组选择育龄女性原发性癫痫间患者40例,对照组选择健康育龄女性29例。于月经周期早卵泡期行彩色多普勒超声检查,并测定血清性激素水平。结果 (1)癫痫间组卵巢动脉收缩期峰流速显著低于对照组(P<0.05);(2)癫痫间组血清雌二醇水平显著高于对照组,而孕酮水平显著低于对照组(P<0.05);癫痫间组窦卵泡计数显著低于对照组(P<0.05);(3)癫痫间组卵巢动脉收缩期峰流速受癫痫间发作频率及血清孕酮水平的明显影响(P<0.05)。结论育龄女性原发性癫痫间患者卵巢动脉收缩期峰流速下降,并受癫痫间发作频率及血清孕酮水平的影响;血清性激素水平异常、窦卵泡计数减少提示卵巢储备功能下降;卵巢动脉血流动力学变化可能为卵巢功能异常的机制之一。
Objective To study the changes of ovarian arterial hemodynamics and sex hormone levels in women of childbearing age with primary epilepsy. Methods Forty patients with primary epilepsy were recruited in epilepsy group and 29 healthy women of childbearing age in control group. During the early menstrual cycle, follicular color Doppler ultrasound examination, and determination of serum sex hormone levels. Results (1) The peak systolic velocity of the ovarian artery in epilepsy group was significantly lower than that in the control group (P <0.05). (2) Serum estradiol level was significantly higher in the interictal epilepsy group than in the control group, while the progesterone level was significantly lower than that in the control group (P <0.05). The count of antral follicles in epilepsy group was significantly lower than that in control group (P <0.05). (3) The peak systolic velocity of epistaxis group was significantly affected by seizure frequency and serum progesterone level <0.05). Conclusions The peak systolic velocity of ovarian artery in women of childbearing age with primary epilepsy is decreased, and the frequency of seizure and the serum progesterone level are affected. The level of serum sex hormones is abnormal. The decrease of antral follicle count indicates the decrease of ovarian reserve. Kinetic changes may be one of the mechanisms of ovarian dysfunction.
