目的:探讨老年急性白血病(Al)患者的免疫表型特点与临床意义。方法:采用流式细胞术对95例老年Al患者进行免疫表型检测,并观察给予化疗后的治疗效果,同时以203例非老年Al患者进行比较。结果:(1)老年急性髓系白血病(AML)患者比例(77.9%)明显高于非老年组AML患者比例(51.7%),差异有统计学意义(P<0.05);(2)老年组AML患者继发于骨髓增生异常综合征(MDS)比例(31.1%)明显高于非老年组比例(4.8%),差别有统计学意义(P<0.05);(3)老年Al患者骨髓原始细胞比例中位数明显低于非老年组((P<0.05));(4)急性髓系白血病中,老年组CD64+(28.7%),CD14+(25.6%)表达率明显高于非老年组CD64+(8.7%),CD14+(9.6%),差异有统计学意义(P<0.05);(5)老年急性白血病患者(Al)完全缓解率(23.7%)明显差于非老年组AL患者(81.5%),差异有统计学意义(P<0.05)。结论:综合分析老年组与非老年组免疫表型特点及临床预后,确定白血病细胞的性质,对老年白血病患者的诊断及治疗预后分析有重要意义;老年白血病患者独特的免疫表型特点可能是老年Al预后不良的原因之一。
Objective: To investigate the immunophenotypic characteristics and clinical significance of elderly patients with acute leukemia (Al). Methods: Flow cytometry was used to detect the immunophenotype of 95 elderly patients with Al and the therapeutic effects after chemotherapy were observed.
