目的探讨女性宫颈细胞中39种人乳头瘤病毒(human papillomavirus,HPV)感染基因型别的分布情况及其临床意义。方法从434例女性宫颈细胞标本中提取39种HPV DNA,采用基因扩增结合基因芯片技术对其细胞进行39种HPV基因型别的检测,并对其患者进行临床资料分析。结果 434例女性宫颈细胞标本中检出HPV阳性者175例,总的HPV感染率40.32%(175/434)。其中单一HPV感染105例,阳性检出率为24.19%(105/434),多重HPV感染70例,阳性检出率为16.13%(70/434)。单一HPV感染中HPV18型为31例,阳性检出率为17.71%(31/175),是最主要的感染型别;其次是HPV16型,共12例,其阳性检出率为6.86%(12/175),HPV52型,为11例,其阳性检出率为6.29%(11/175)。多重HPV感染中,HPV6+54型、HPV18+52型和HPV51+68型各2例,各占多重感染的2.86%(2/70),它们是多重HPV感染的最主要型别。结论 HPV16型、18型、52型、6+54型、18+52型和51+68型感染是女性宫颈细胞的主要感染类型。基因扩增结合基因芯片技术是一种适合临床开展HPV分型诊断和行HPV感染的分子流行病学研究的方法。随着HPV检测型别的增加,HPV感染率也随之增加,其基因组合型别更趋多样化。
Objective To investigate the distribution of 39 human papillomavirus (HPV) genotypes in female cervical cells and its clinical significance. Methods 39 kinds of HPV DNA were extracted from 434 female cervical cell samples. 39 HPV genotypes were detected by gene amplification and gene chip technology, and their clinical data were analyzed. Results A total of 175 HPV positive cases were detected in 434 cases of female cervical cells, the total HPV infection rate was 40.32% (175/434). The single HPV infection in 105 cases, the positive detection rate was 24.19% (105/434), multiple HPV infection in 70 cases, the positive detection rate was 16.13% (70/434). In the single HPV infection, 31 cases were HPV18 and the positive rate was 17.71% (31/175), which was the most common type of infection. HPV16 was the second most common type, with a total of 12 cases. The positive rate was 6.86% (12 / 175), HPV52 type, 11 cases, the positive detection rate was 6.29% (11/175). Among the multiple HPV infections, HPV6 + 54, HPV18 + 52 and HPV51 + 68 were 2 (2.86%) each, accounting for 2.86% (2/70) of the multiple infections, respectively. They were the most common type of multiple HPV infection. Conclusion HPV16, 18, 52, 6 + 54, 18 + 52 and 51 + 68 infection are the main types of cervical cell infection in women. Gene amplification combined with gene chip technology is a suitable clinical diagnosis of HPV typing and HPV infection in molecular epidemiological methods. With the increase of HPV detection type, HPV infection rate also increases, its genotypes more diversified.
