目的:探讨肝胆特异性对比剂Gd-EOB-DTPA在磁共振胆道成像中的最佳延迟时间。方法:采用轴面容积式内插值法屏气检查(VIBE)序列,对31例肝功能正常的志愿者行上腹部Gd-EOB-DTPA增强MRI动态扫描,获取5、10、15、20、25、30、40、50、60、70、80、90min轴面图像。测量胆道系统各部位(左肝管、右肝管、肝总管、胆囊、胆囊管及胆总管(近端、中段、远端))的信号强度(SI),计算相应时间点胆道系统各部位的信噪比(SNR)和对比噪声比(CNR),描绘其变化趋势曲线并记录达峰时间点。采用Likert 5级评分法分别评价各个时间点左右肝管一二级分支、肝总管、胆囊、胆囊管及胆总管的可视性。结果:胆囊的SI、SNR、CNR的达峰时间均为70~80min;左右肝管、肝总管、胆囊管及胆总管(近端、中段、远端)SI出现峰值的时间为40~50min,SNR出现峰值的时间为50~60min;胆囊管、左右肝管、肝总管及胆总管近端CNR出现峰值的时间为30~40min,胆总管中段和远端CNR出现峰值的时间为60~70min。结论:肝功能正常成人胆囊最佳延迟时间为注射Gd-EOB-DTPA后80min,左右肝管、肝总管、胆囊管及胆总管的最佳延迟强化时间为40~60min。
Objective: To investigate the optimal delay time of hepatobiliary-specific contrast agent Gd-EOB-DTPA in magnetic resonance cholangiography. Methods: Thirty-one volunteers with normal hepatic function underwent Gd-EOB-DTPA enhanced MRI dynamic scanning in the abdomen with the axial volumetric interpolated breath hold test (VIBE) sequence, and got 5,10,15,20,25 30, 40, 50, 60, 70, 80, 90min axial image. The signal intensity (SI) of various parts of the biliary tract (left hepatic duct, right hepatic duct, hepatic duct, gallbladder, cystic duct and common bile duct (proximal, middle and distal)) was measured and the biliary system Signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) are plotted against the trend curve and the peak time points are recorded. Likert 5-grade scoring method was used to evaluate the visibility of primary and secondary hepatic duct branches, hepatic duct, gallbladder, cystic duct and common bile ducts at each time point. Results: The peak time of SI, SNR and CNR of the gallbladder were both 70-80min. The peak time of SI in the hepatic duct, hepatic duct, cystic duct and common bile duct (proximal, middle and distal) was 40-50min, SNR peak time of 50 ~ 60min; cystic duct, left and right hepatic duct, hepatic duct and common bile duct proximal CNR peak time was 30 ~ 40min, the common bile duct CNR and distal peak time was 60 ~ 70min. CONCLUSION: The optimal delay time of gallbladder with normal liver function is 80min after injection of Gd-EOB-DTPA, and the optimal delay-strengthening time of liver, hepatic, cystic and common bile ducts is 40 ~ 60min.
