目的:观察中医内服外贴联合利福喷丁对肺结核患者呼吸功能相关指标及细胞炎症因子水平的影响。方法:选取2014年1月~2014年12月我院感染性疾病科结核门诊收治的86例肺结核患者为研究对象,随机数字表法分为两组各43例,其中对照组给予利福喷丁治疗,观察组在对照组基础上另联合中医内服外贴治疗,观察治疗后两组临床疗效比较及治疗前、后两组呼吸功能相关指标[用力肺活量(FVC)、第一秒用力呼气容积(FEV1)、最大通气量(MVV)]比较,并观察两组治疗前、后细胞炎症因子[白细胞介素-6(IL-6)、白细胞介素-10(IL-10)、肿瘤坏死因子(TNF-α)]水平比较、治疗后痰菌阴转情况及治疗后不良反应比较。结果:治疗后观察组总有效率显著高于对照组(P<0.05);两组呼吸功能指标分别均较治疗前显著升高,且观察组FVC%、FEV1%、MVV%较对照组升高更明显,具有统计学意义(P<0.05);两组细胞炎症因子水平分别均较治疗前显著降低,且观察组IL-6、L-10、TNF-α较对照组降低更显著,具有统计学意义(P<0.05);两组治疗后痰菌阴转率差异不显著(P>0.05),治疗后观察组不良反应总发生率9.30%较对照组32.56%显著低(P<0.05)。结论:中医内服外贴联合利福喷丁对肺结核患者具有较好的临床疗效,可显著升高患者呼吸功能相关指标,显著降低炎症因子相关指标,该方案安全可靠,在临床中具有较广泛的应用价值。
Objective: To observe the effects of Chinese medicine oral administration of combination of rifapentine on respiratory function and the level of inflammatory cytokines in pulmonary tuberculosis patients. Methods: From January 2014 to December 2014, 86 patients with tuberculosis admitted to tuberculosis clinic of infectious diseases department of our hospital were enrolled in this study. The random number table was divided into two groups of 43 cases. The control group was given rifapentine After treatment, the observation group was combined with the external application of traditional Chinese medicine (TCM) on the basis of the control group, and the clinical efficacy of the two groups were observed and compared before and after treatment [FVC (forced expiratory volume) (FEV1) and maximum ventilation (MVV)]. The levels of inflammatory cytokines (interleukin-6, interleukin-10, tumor necrosis factor (TNF-α)] levels, after treatment, sputum negative conversion and adverse reactions after treatment compared. Results: After treatment, the total effective rate of the observation group was significantly higher than that of the control group (P <0.05). The respiratory function indexes of the two groups were significantly higher than those before treatment, and the FVC%, FEV1% and MVV% of the observation group were higher than those of the control group (P <0.05). The levels of inflammatory cytokines in both groups were significantly lower than those before treatment, and the levels of IL-6, L-10 and TNF-α in the observation group decreased more significantly than those in the control group (P0.05) .There was no significant difference in sputum negative conversion rate between the two groups after treatment (P> 0.05). After treatment, the overall incidence of adverse reactions in the observation group was significantly lower than that in the control group (9.30% vs 32.56%, P <0.05). Conclusion: Chinese medicine oral administration of combination of rifampicin on patients with pulmonary tuberculosis has a good clinical efficacy, can significantly increase the respiratory function-related indicators, significantly reduce the relevant indicators of inflammatory cytokines, the program safe and reliable, has a wide range of clinical Value.
