根据腺管形成、细胞分化及核分裂相将乳腺癌分为三级;浆细胞反应以每个毛细淋巴管周围浆细胞数在5个以上为阳性,基底膜破环与弹力纤维反应也分为三级。观察145例乳腺癌,结果发现:乳腺癌Ⅰ级59例,其中浆细胞反应阳性38例,占64.4%;乳腺癌Ⅱ级67例,浆细胞反应阳性30例,占44.8%;乳腺癌Ⅲ级19例,浆细胞反应阳性4例,占21.1%。说明浆细胞反应阳性与乳腺癌分级数呈负相关。而各级乳腺癌中,浆细胞反应阴性病例的淋巴结转移分别为66.7%,68.4%,78.5%,说明浆细胞反应程度直影响乳腺癌的淋巴结转移。纤维反应(+)者13例,淋巴结阳性3例,占23.1%,纤维反应(++)者52例,淋巴结阳性28例,占53.8%;纤维反应(+++)者59例,淋巴结阳性35例,占69.5%。淋巴结转移率与纤维反应分级数呈正相关,从而提示:乳腺癌基底膜破环,弹力纤维增生、变性、固缩均可作为乳腺癌的预后指标。

At all levels of breast cancer, lymph node metastasis was negative in 66.7%, 68.4%, and 78.5%, respectively, indicating that the degree of plasma cell response directly affected the lymph node metastasis of breast cancer. There were 13 patients with fibrosis (+), 3 with positive lymph nodes (23.1%), 52 with fibrosis (++), 28 with lymph nodes (53.8%), 59 with fibrosis (+++). Positive in 35 cases, accounting for 69.5%. The rate of lymph node metastasis was positively correlated with the number of grades of fiber reaction, suggesting that the basement membrane of breast cancer is destroyed and hyperplasia, degeneration, and contraction of elastic fibers can be used as prognostic indicators of breast cancer.
