一位50岁男性患者因左耳部分鼓膜穿孔和中耳化脓性感染而应用庆大霉素滴耳剂治疗6周,停药1周后,患者突然发生体位性眩晕及共剂失调。患者每于起坐或快速转头时发生眩晕持续20～30秒,并伴有恶心、共济失调和左侧耳呜。 耳神经病学测验表明,眼震电图的一系列改变与体位性眼震和温度试验反应降低相符合,左耳呈重度感觉神经性聋(4000赫45分贝,8000赫40分贝)。
A 50-year-old man was treated with gentamicin ear drops for 6 weeks due to perforation of the tympanic membrane of the left ear and purulent infection of the middle ear. One week after discontinuation of the drug, the patient developed sudden vertigo and comorbid disorders. Patients with vertigo every 20 minutes or 30 seconds during sitting or rapid turnaround, accompanied by nausea, ataxia, and left atrial appearing. Otology tests have shown that a series of changes in the electro-oculography are consistent with reductions in posture nystagmus and temperature test responses, with severe sensory deafness in the left ear (4000 Hz 45 dB, 8000 Hz 40 dB).
