目的:探讨彩色多普勒超声检查诊断甲状腺结节的意义,并分析其诊断优势。方法:选取我院收治的88例甲状腺结节的患者作为研究对象,所有患者均在治疗前实施彩色多普勒超声检查,回顾其影像学诊断结果,根据图像特征判断结节的良恶性,比较两者的区别,同时,与最终临床诊断结果进行对照,计算超声检查的符合率。结果:经最终病理诊断,共检出甲状腺囊肿、结节性甲状腺肿、甲状腺瘤、甲状腺癌患者各15例、32例、29例、12例,超声诊断符合率分别为93.3%、96.9%、93.1%、91.7%,各种不同类型甲状腺疾病的诊断符合率比较差异不大(P>0.05),总的诊断符合率为94.3%。对确诊的良性结节和恶性结节的超声图像特征进行观察比较,可见,恶性结节的内部低回声、合并微钙化、边界前清洗、形态不规则的比例以及纵横比、RI、EDV值的测定值均显著高于良性结节,而PSV测定值则显著低于良性结节,比较均有统计学差异(P<0.05)。结论:彩色多多普勒超声可以对甲状腺结节的性质进行鉴别,为不同类型甲状腺疾病的分型提供依据,指导患者的临床治疗,加上其操作简便,安全性好,经济性佳,准确率高,值得临床推广应用。
Objective: To explore the significance of color Doppler ultrasonography in the diagnosis of thyroid nodules and to analyze its diagnostic advantages. Methods: Eighty-eight patients with thyroid nodules admitted to our hospital were enrolled in this study. All patients underwent color Doppler ultrasonography before treatment. The diagnostic results of imaging were reviewed. The nodules’ benign and malignant characteristics were evaluated according to the image characteristics. The difference between the two, at the same time, compared with the final clinical diagnosis, calculate the coincidence rate of ultrasound. Results: The final diagnosis of thyroid cyst, nodular goiter, thyroid tumor and thyroid cancer in 15 cases, 32 cases, 29 cases, 12 cases, the diagnostic accuracy of ultrasound was 93.3%, 96.9%, respectively, 93.1% and 91.7% respectively. There was no significant difference in the diagnostic coincidence rates among various types of thyroid diseases (P> 0.05). The overall diagnostic coincidence rate was 94.3%. The echocardiographic features of benign and malignant nodules were observed and compared. It was found that the internal hypoechoicity of malignant nodules, combined microcalcification, pre-boundary cleaning, irregular shape and aspect ratio, RI, EDV The measured values were significantly higher than those of benign nodules, while the values of PSV were significantly lower than those of benign nodules (P <0.05). Conclusion: The color Doppler ultrasound can identify the nature of thyroid nodules and provide the basis for the classification of different types of thyroid diseases, and guide the clinical treatment of patients, coupled with its simple, safe, good economy, accuracy High, it is worth promoting clinical application.
