目的分析阑尾周围脓肿穿刺脓液的细菌培养及药敏试验结果。方法选取2012年10月—2014年10月于山西省潞城市人民医院行B超引导下脓肿穿刺引流治疗的阑尾周围脓肿患者100例,将患者脓肿穿刺引流所得的脓液标本进行细菌培养及药敏试验。结果 100例送检的标本中检出细菌79例(79.0%);分离得到的细菌84株,其中革兰阴性菌69株(82.1%),革兰阳性菌15株(17.9%);需氧菌72株(85.7%),厌氧菌12株(14.3%)。单一细菌感染患者27例,两种细菌混合感染患者44例,多种细菌混合感染患者13例。大肠埃希菌对亚胺培南、美洛培南、丁胺卡那霉素、头孢西丁、哌拉西林-他唑巴坦的敏感性较高;肺炎克雷伯菌对亚胺培南、美罗培南、丁胺卡那霉素、头孢西丁的敏感性较高;铜绿假单胞菌对亚胺培南、美罗培南、丁胺卡那霉素、头孢西丁、哌拉西林-U的敏感性较高。结论阑尾周围脓肿的致病菌主要为革兰阴性菌,临床应重视细菌培养及药敏试验结果,并根据结果合理选择抗生素。
Objective To analyze the bacterial culture and drug susceptibility test results of abscess puncture pus surrounding the appendix. Methods From October 2012 to October 2014 in Lucheng People’s Hospital of Shanxi Province, 100 cases of peri-abscessed abscesses were treated with B-guided abscess drainage. The specimens of patients with abscess were collected for bacterial culture and drug Sensitive test. Results 79 strains (79.0%) were detected in 100 specimens tested. 84 strains of bacteria were isolated, of which 69 (82.1%) were Gram-negative and 15 (17.9%) were Gram- 72 strains of bacteria (85.7%), 12 strains of anaerobic bacteria (14.3%). 27 cases of single bacterial infection, 44 cases of two kinds of bacterial mixed infection, 13 cases of multiple bacterial mixed infection. Escherichia coli is more sensitive to imipenem, meropenem, amikacin, cefoxitin and piperacillin-tazobactam; Klebsiella pneumoniae is sensitive to imipenem , Meropenem, amikacin, and cefoxitin were more sensitive. Pseudomonas aeruginosa had higher sensitivity to imipenem, meropenem, amikacin, cefoxitin, piperacillin-U The higher the sensitivity. Conclusion The pathogenic bacteria of abscess around appendix are mainly Gram-negative bacteria. Clinic culture should pay attention to bacterial culture and drug susceptibility test results, and choose antibiotics reasonably according to the result.
