目的了解江苏省高邮市医疗卫生单位门诊患者卫生服务需求与利用的相关影响因素,为农村卫生适宜技术推广及医疗保障制度的施行提供科学信息参考。方法采用统一问卷,从高邮市医疗卫生单位抽取5家医院进行门诊患者一日现况调查。结果有效问卷的相关分析显示:44.3%的门诊患者就诊费用在100元以下,100~200元的占到30.1%,300元以上的占到15.4%,200~300元的人数最少(10.2%)。就诊费用与调查对象的性别、就业状况、就诊医院级别、科室、疾病类别及药费相关。55.1%的门诊患者认为就诊流程简易,41.7%的认为一般,认为复杂的有3.1%;对医生的技术水平评价中,好评的占89.5%,其他10.5%的人认为一般。患者对医生技术水平评价与就诊医院级别、能否报销、疾病类别及年龄相关。结论应针对不同人群的特点,在不同级别的医院,综合各种影响因素,广泛开展新型农村合作医疗等医疗保障制度,合理选择卫生适宜技术,提高基层医院医疗服务质量,降低医疗费用,真正解决农民看病难、看病贵的问题。
Objective To understand the related factors that affect the demand and utilization of outpatients’ health services in medical and health units in Gaoyou, Jiangsu Province, and provide scientific information for the popularization of appropriate technology in rural areas and the implementation of medical security system. Methods Using the unified questionnaire, five hospitals were selected from the medical and health units of Gaoyou for the first day survey of outpatients. Results Analysis of relevant valid questionnaires showed that 44.3% of outpatients had less than 100 yuan for medical treatment, 30.1% for 100-200 yuan, 15.4% for more than 300 yuan and 10.2% for those with 200-300 yuan . The cost of the visit is related to the sex of the respondent, employment status, hospital level, department, type of illness and medical expenses. 55.1% of the outpatients considered the treatment procedure as simple, 41.7% considered it as complicated and 3.1% as complicated, 89.5% rated the doctor’s skill, and 10.5% considered it normal. The patient’s evaluation of the technical level of the doctor is related to the hospital level, reimbursement, disease type and age. Conclusion According to the characteristics of different groups of people, in different levels of hospitals, we should comprehensively carry out various medical insurance systems, such as new rural cooperative medical care, appropriately select suitable health technologies, improve the quality of medical service in primary hospitals and reduce the medical expenses, so as to really solve the problems Farmers see a doctor difficult to see expensive problem.
