目的研究安尔碘、复合双链季铵盐和戊二醛三种常用消毒剂对院内感染病原菌杀菌效果。方法采集患者痰液、分泌物、脓液、血、便等样本进行培养,并进行菌株分离和鉴定。选取菌株数居前3位病原菌进行药敏试验,判读标准美国临床实验室标准化委员会药敏试验标准2015版。选取多重耐药菌各10株进行MIC和MBC测定。结果分离病原菌180株,其中革兰阴性菌104株,革兰阳性菌76株,未检测到真菌;铜绿假单胞菌37株、大肠埃希菌28株、金黄色葡萄球菌27株数量。病原菌标本来源:痰液、咽拭子71株,伤口、创面分泌物52株,脓液21株,血液16株,枕头、被褥9株,空调开关5株,其他6株。铜绿假单胞菌对氨卡西林、青霉素、头孢噻吩耐药率分别为100%、86.49%、100%,大肠埃希菌和金黄色葡萄球菌对上述3种抗生素全部耐药100%。安尔碘、复合双链季铵盐和戊二醛三种消毒剂对多重耐药菌MIC测定结果:铜绿假单胞菌安尔碘MICs值高于128mg/L的有6株,复合双链季铵盐MICs值高于128mg/L的有3株,戊二醛MICs值高于128mg/L的有5株。而金黄色葡萄球菌安尔碘MICs值高于128mg/L的有2株,戊二醛MICs值高于128mg/L的有1株,未发现有复合双链季铵盐MICs值高于128mg/L的菌株。MBC测定结果:铜绿假单胞菌安尔碘、复合双链季铵盐和戊二醛MBCs分别有3株,1株和3株超过1 024mg/L。金黄色葡萄球菌对上述3种消毒剂的最高MBCs仅为512mg/L。结论由于不同成分消毒剂杀毒机制不同,因而轮换使用消毒剂可以取得较好的杀菌效果。
Aim To study the bactericidal effect of three common disinfectants such as arsenic trioxide, complex double-stranded quaternary ammonium salt and glutaraldehyde on nosocomial infection pathogens. Methods Samples of sputum, secretions, pus, blood, urine and other samples were collected for culture, and strains were isolated and identified. Select the top three strains of bacteria susceptibility testing, interpretation standard American Clinical Laboratory Standardization Committee susceptibility test standard 2015 version. Select 10 strains of multi-resistant bacteria for MIC and MBC determination. Results 180 strains of pathogens were isolated, of which 104 strains were Gram-negative bacteria and 76 strains were Gram-positive bacteria. No fungi were detected; 37 strains were Pseudomonas aeruginosa, 28 strains were Escherichia coli and 27 were Staphylococcus aureus. Pathogenic bacteria specimens source: sputum, throat swab 71, wounds, wound secretions 52, 21 pus, blood 16, pillow, bedding 9, air conditioning switch 5, the other 6. The resistance rates of Pseudomonas aeruginosa to amikacin, penicillin and cefalotin were 100%, 86.49% and 100%, respectively. Escherichia coli and Staphylococcus aureus were 100% resistant to the three antibiotics. Arsenic trioxide, complex double chain quaternary ammonium salt and glutaraldehyde three kinds of disinfectant MIC multi-resistant test results: Pseudomonas aeruginosa Aner iodine MICs higher than 128mg / L of 6, double-stranded There were 3 strains with quaternary ammonium salt above 128 mg / L and 5 strains with higher glutaraldehyde than 128 mg / L. While Staphylococcus aureus Aner iodine MICs higher than 128mg / L of 2 strains, glutaraldehyde MICs higher than 128mg / L of a strain, did not find a compound double-stranded quaternary ammonium MICs value higher than 128mg / L strain. The results of MBC assay showed that there were 3 strains, 1 strain and 3 strains more than 1 024 mg / L, respectively, of Pseudomonas aeruginosa Anergine, complex double-stranded quaternary ammonium salt and glutaraldehyde MBCs. Staphylococcus aureus on the three kinds of disinfectants, the highest MBCs only 512mg / L. Conclusion As different components of disinfectant anti-virus mechanism is different, so the use of disinfectant rotation can achieve better bactericidal effect.
