目的:观察经导管股动脉多次灌注丹参多酚酸盐联合球囊血管介入治疗糖尿病足的临床疗效。方法:将90例糖尿病足患者随机分为3组,联合介入组行下肢血管造影加球囊扩张成形术治疗,术中术后予注射用丹参多酚酸盐等治疗,纯介入组仅予球囊扩张成形术治疗,非介入组不采用介入方法。3组均观察2年。结果:联合介入组的疗效优于纯介入组和非介入组(P<0.05,P<0.01)。治疗后,3组各时段的踝/臂血压指数比值(ABI)、CT血管造影(CTA)动脉管腔情况均较治疗前改善(P<0.05,P<0.01),联合介入组ABI、CTA动脉管腔情况的改善优于纯介入组与非介入组(P<0.05,P<0.01),纯介入组ABI、CTA动脉管腔情况的改善均优于非介入组(P<0.01)。联合中药介入组截肢率均低于纯介入组与非介入组(P<0.05,P<0.01),纯介入组截肢率低于非介入组(P<0.05)。结论:经导管股动脉多次灌注丹参多酚酸盐联合球囊血管介入治疗糖尿病足临床疗效显著,血管再狭窄率与截肢率明显降低。
OBJECTIVE: To observe the clinical effect of multiple perfusion of Danshen polyphenols combined with balloon angioplasty on diabetic foot via catheter femoral artery. Methods: Ninety patients with diabetic foot were randomly divided into three groups. The interventional group was treated with lower extremity angiography and balloon dilatation and angioplasty. After intraoperative and postoperative injection of Salviae miltiorrhizae polyphenols, Balloon dilation, non-intervention group does not use interventional methods. 3 groups were observed for 2 years. Results: The efficacy of the combined intervention group was better than that of the intervention group and the non-intervention group (P <0.05, P <0.01). After treatment, the ankle / arm index (ABI) and the lumen of CT angiography (CTA) were improved in all three groups (P <0.05, P <0.01) The improvement of lumen was better than that of the intervention group and the non-intervention group (P <0.05, P <0.01). The improvement of the lumen of ABI and CTA in the intervention group was better than that in the non-intervention group (P <0.01). The amputation rate of the intervention group was lower than that of the intervention group and the non-intervention group (P <0.05, P <0.01). The amputation rate of the intervention group was lower than that of the non-intervention group (P <0.05). CONCLUSION: The multi-infusion of Salviae miltiorrhizae polyphenols and balloon angioplasty in the treatment of diabetic foot through the catheter femoral artery has significant curative effect, and the rate of vascular restenosis and amputation rate are significantly reduced.
