目的 探讨急性缺血性脑卒中(AIS)患者外周血基质细胞衍生因子-1(SDF-1)水平的动态变化,并分析其与AIS预后的关系.方法 选择AIS患者92例(AIS组)及具有高危因素的非AIS患者20例(Risk组),采用酶联免疫吸附试验(ELISA)测定SDF-1水平,应用改良Rankin量表(mRS)评估AIS患者发病90天后功能恢复结局,按预后情况将其分为预后良好组(71例)和预后不良组(21例),并对其SDF-1水平进行比较分析.结果 与Risk组相比,AIS组患者SDF-1水平在入院第1天和发病第7天均明显升高,且SDF-1表达量在发病第7天明显高于入院第1天,差异有统计学意义(P<0.01).在AIS患者中,与预后不良组比较,预后良好组SDF-1水平在发病第7天升高(P<0.05),且发病第7天显著高于入院第1天(P<0.01).SDF-1的变化率与90天mRS评分相关(t=1.211,P=0.044).结论 AIS患者急性期外周血中SDF-1发生了动态变化,其变化率与患者90天的mRS具有相关性,提示外周血SDF-1的水平可以作为AIS预后的预测因素.“,”Objective To study the dynamic change of stromal cell-derived factor 1 (SDF-1) level of acute ischemic stroke (AIS) in peripheral blood and the correlation of SDF-1 level with the outcome of AIS.Methods A total of 92 AIS patients and 20 patients with risk factors were recruited.SDF-1 in the plasma was measured by enzyme linked immunosorbent assay(ELISA).Functional recovery was assessed at the 90th day after AIS by modified Rankin Scale (mRS).AIS patients were divided into good prognosis group(71 cases) and poor prognosis group(21 cases).Compare and analyze the SDF-1 levels between two groups.Results Compared with the Risk group,SDF-1 level in patients with AIS on the first day of admission and on the 7th day of illness were both higher.The level of SDF-1 on the 7th day of illness was significantly higher than that of the first day of admission,and the differences were statistically different(P < 0.01).Within AIS patients,compared with the poor prognosis group,SDF-1 level of good prognosis group on the 7th day of illness rose(P < 0.05),which was significantly higher than that of the first day(P < 0.01).The change rate of SDF-1 was correlated with the mRS scores on the 90th day after AIS (t =1.211,P =0.044).Conclusion There exist dynamic changes of SDF-1 level in peripheral blood of patients with AIS,which are related to the mRS scores of 90th day after AIS.SDF-1 level could be used as the predictor of the prognosis of AIS.
