目的 探讨缬沙坦联合氨氯地平对老年高血压患者血管内皮功能及日常生活能力的影响。方法 选取2014年11月—2016年11月襄城县人民医院78例老年高血压患者,依据建档顺序分为观察组和对照组,每组39例。两组均予以常规治疗,对照组于此治疗基础上仅采用缬沙坦治疗,观察组采用缬沙坦联合氨氯地平治疗,两组均治疗2个月。统计比较两组治疗前、治疗2个月时血管内皮功能指标[一氧化氮(NO)、内皮素-1(ET-1)]水平、日常活动能力(BI)评分并评估两组临床疗效。结果 观察组治疗2个月时NO水平较对照组明显升高,ET-1水平较对照组降低,差异有统计学意义(P<0.05);观察组治疗2个月时BI评分较对照组升高,差异有统计学意义(P<0.05)。经治疗,观察组总有效率为84.62%(33/39),高于对照组64.10%(25/39),差异有统计学意义(P<0.05)。结论 老年高血压患者应用缬沙坦联合氨氯地平治疗降压效果显著,且可明显改善患者血管内皮功能,提高其日常生活能力。
Objective To investigate the effects of valsartan combined with amlodipine on vascular endothelial function and daily living ability in elderly patients with essential hypertension. Methods From November 2014 to November 2016, 78 elderly patients with hypertension in Xiangcheng County People’s Hospital were divided into observation group and control group according to the order of filing, with 39 cases in each group. The two groups were given conventional treatment, the control group on the basis of this treatment only with valsartan treatment, the observation group with valsartan combined with amlodipine treatment, both groups were treated for 2 months. The levels of endothelial nitric oxide (NO) and endothelin-1 (ET-1) and the activity of daily activities (BI) were compared between the two groups before treatment and 2 months after treatment, and the clinical efficacy was evaluated. Results The level of NO in the observation group was significantly higher than that in the control group at 2 months and the ET-1 level was lower than that in the control group (P <0.05). The BI score in the observation group was significantly higher than that in the control group at 2 months High, the difference was statistically significant (P <0.05). After treatment, the total effective rate of the observation group was 84.62% (33/39), which was higher than that of the control group (64.10%, 25/39). The difference was statistically significant (P <0.05). Conclusions Valsartan combined with amlodipine can significantly decrease the blood pressure in elderly hypertensive patients, and can significantly improve the vascular endothelial function and improve their daily living ability.
