目的探讨急性髓性白血病(AML)患者年龄与P-糖蛋白(P-gp)表达及功能的关系。方法2001—2004年将中国医科大学附属第一医院31例初治AML患者分为≥60岁(高龄)和<60岁(低龄)两组,用免疫组化、逆转录聚合酶链反应(RT-PCR)检测P-gp表达,用流式细胞术检测P-gp功能,比较两组间的差异。结果免疫组化和RT-PCR检测显示,12例高龄患者中6例P-gp表达阳性,占50.0%;19例低龄患者中5例P-gp表达阳性,占26.3%,两组间差异无显著性意义(P>0.05)。12例高龄患者中7例P-gp功能阳性,占58.3%;19例低龄患者中3例P-gp功能阳性,占15.8%,高龄组P-gp功能阳性率高于低龄组(P<0.05);P-gp的表达与功能呈正相关(γ=0.784,P=0.001)。结论高龄AML患者P-gp功能阳性率高于低龄组;对高龄AML患者P-gp功能的检测更具有临床指导意义。
Objective To investigate the relationship between the age and the expression of P-glycoprotein (P-gp) and its function in patients with acute myeloid leukemia (AML). Methods A total of 31 newly diagnosed AML patients from the First Affiliated Hospital of China Medical University from 2001 to 2004 were divided into two groups (≥60years old and <60years old). Immunohistochemistry, reverse transcription polymerase chain reaction -PCR) was used to detect the expression of P-gp, the function of P-gp was detected by flow cytometry, and the differences between the two groups were compared. Results Immunohistochemistry and RT-PCR showed that the expression of P-gp was positive in 6 of 12 elderly patients (50.0%). The positive expression of P-gp was found in 5 of 19 young patients (26.3%), with no difference between the two groups Significant significance (P> 0.05). P-gp function was positive in 7 of 12 elderly patients, accounting for 58.3%. Three of the 19 younger patients had positive P-gp function, accounting for 15.8%. The positive rate of P-gp in advanced age group was higher than that in younger age group (P <0.05) ). The expression of P-gp was positively correlated with the function (γ = 0.784, P = 0.001). Conclusion The positive rate of P-gp in elderly patients with AML is higher than that in younger patients. The detection of P-gp function in elderly patients with AML is more clinical significance.
