目的探讨超声检查在产前筛查和诊断胎儿畸形中的临床运用价值。方法应用彩色多普勒超声对1 600名中晚孕龄胎儿进行全面的系统畸形检查,发现畸形者,再反复核对,严重畸形或多系统畸形者,建议引产、尸检。结果检出单一系统和多系统畸形89例。其中神经系统畸形17例,泌尿系统畸形5例,消化系统畸形4例,循环系统畸形20例,双胎畸形3例,其他畸形40例,其中61例引产后尸检证实,28例出生后彩色多普勒超声复查证实,1例Ⅲ度唇裂及2例复杂先天性心脏病因胎儿条件不合适未检出。1例显性脊柱裂漏诊。结论二维彩色多普勒超声是诊断胎儿单一或多系统明显畸形的良好方法。
Objective To investigate the value of ultrasonography in prenatal screening and diagnosis of fetal malformations. Methods A total of 1,600 fetuses with middle and late gestational age were examined by color Doppler ultrasonography and found to have deformity, repeated checkups, severe deformities or multiple system deformities. It is recommended to induce labor and autopsy. Results A single system and multiple system abnormalities were detected in 89 cases. Among them, 17 cases of nervous system deformity, 5 cases of urinary system deformity, 4 cases of digestive system deformity, 20 cases of circulatory system deformity, 3 cases of twins deformities and 40 cases of other deformities, of which 61 post-abortion autopsy confirmed that 28 cases were born after color Pulsed ultrasound confirmed that one case of Ⅲ degree cleft lip and 2 cases of complicated congenital heart disease due to fetal conditions are not detected. One case of dominant spina bifida missed diagnosis. Conclusion Two-dimensional color Doppler ultrasound is a good method to diagnose the obvious deformity of single or multiple system of fetus.
